Job description

General Summary of PositionAssesses and provides nutrition treatment, consultation, and /or education to a primarily specialized group of patients and/or families as ordered by physicians.
Primary Duties and Responsibilities
  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Provides quality diabetes education to patients, families and groups. Serves as an education resource to the institution, the staff, and the community.
  • Consults with and/or makes recommendations to physicians and other allied health professionals, regarding patients nutritional status, nutrient needs or specific diet plan.
  • Formulates nutritional assessment, evaluation, and follow-up for patients at high risk (comprehensive are level) or according to departmental policy for patients who may be at risk and recommends patient's nutrition treatment plans findings in patients medical record.
  • Consults participants in the Diabetes Self Management Education program in diet rationale for self-management.
  • May be responsible for: Documents relevant aspects of diabetes care for patient in the medical chart, meets MDI, ADA and CMS standard of chart content, and prepares and communicates documentation to referring physician. Bills for group and individual patient encounters, maintaining billing and documentation requirements and standards.
  • Communicates effectively with healthcare team members regarding patient care for individuals at risk. Serves as a nutrition resource to the other members of the professional staff.
  • Participates in the developments of curriculum, handouts, protocols, and visual aids. Coordinates with the multidisciplinary team.
  • Participates in marketing and coordinating diabetes education resources. Develops and implements marketing strategies to effectively promote the utilization of the Diabetes Education Program to physicians, patients, and the public.
  • Maintains professional licensure and certification in area of clinical expertise. Seeks educational experience to maintain current knowledge of trends in care of diabetic patients. Attends pertinent meeting to enhance clinical practice to support growth, recorded and documented in CDR Professional Development Plan.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.
  • May be required to operate/transport nutrition vehicle to community partners and clinic sites.
  • Establishes data collection and management protocols for tracking program goals and metrics.

  • Minimum Qualifications
    Education
    • B.S. degree with a major in Food & Nutrition from accredited college required or
    • university which includes an accredited dietetic internship coupled with a minimum of 5 years progressive clinical nutrition experience, required or
    • an M.S. degree in food and nutrition with approved training from the American Dietetic Association coupled with 3 years clinical nutrition experience, required or
    • Specialty certification acquired such as CNSD, CDE, required or
    • other specialty which will benefit GUH nutrition practice and three years clinical nutrition experience required or
    • CONSIDERATION WILL BE GIVEN TO AN APPROPRIATE COMBINATION OF EDUCATION/TRAINING AND EXPERIENCE. required
    • One year of relevant education may be substituted for one year of required work experience.

    Experience
    • 3-4 years Clinical nutrition experience with M.S. degree in food and nutrition with approved training from the american dietetic association required or
    • 3-4 years Clinical nutrition experience with specialty certification such as CNSD, CDE, etc. required or
    • 5-7 years Progressive clinical nutrition experience with B.S. degree with a major in food & nutrition from accredited college or university which includes an accredited dietetic internship required

    Licenses and Certifications
    • REG DIET - Dietitian - Registered Registered by the American Dietetic Association required or
    • American Dietetic Assoc - ADA Academy of Nutrition and Dietetics, Licensed in the District of Columbia. required

    Knowledge, Skills, and Abilities
    • Current knowledge of clinical dietetics;
    • Effective verbal and written communication skills;
    • Flexibility in schedule and tasks, as needed;
    • Computer skills;
    • Legible handwriting;
    • good customer service skills and attitude, team player.
