Use the Person Centered Planning Process to develop supportive strategies to help meet the needs and interests of people with intellectual and developmental disabilities; and improve their physical, mental, social, and emotional health while enhancing their ability to live valued roles and participate fully in the community. Schedule regular visits and promote positive relations with consumers, family, guardians, and advocacy agencies. Maintain accurate and timely consumer records, progress notes, medical records, emergency information, and assessments. Full-time position. Excellent compensation and benefits.
Requirements: Must be willing to travel throughout Penobscot county as required for client visits. Must have at least one years work or other relevant experience with people with developmental disabilities. Bachelor’s Degree in Social Work, Human Services, or relevant degree and experience. Must have a valid and clean Maine driver’s license.
