Rehab Services is seeking a dedicated and compassionate Community Care Coordinator to join our team. In this role, you will provide consumer-driven and family-oriented services to our clients. You will work closely with our Licensed Mental Health Professionals (LMHP) and other team members to ensure the highest quality of care. Responsibilities- Provide strength-based, solution-focused, client-centered supportive therapeutic services in the community, including crisis intervention.
- Assist LMHP in completion of initial and subsequent authorization/treatment plan paperwork.
- Help complete intake and annual assessment paperwork in the community or office setting with oversight by the LMHP.
- Assist in completing all discharge paperwork to be reviewed by LMHP/Corporate Compliance.
- Responsible for after-hours on-call duties.
- Submit all paperwork to LMHP/Office Manager by established deadlines.
- Maintain assigned number of billable hours per week.
- Document services provided concisely and completely in accordance with clinical standards by office deadline.
- Available for supervision meetings, trainings, and staffing meetings as scheduled by Clinical Manager.
- Return phone calls and emails promptly.
- Obtain 20 training hours as required for a full-time staff member yearly.
- Actively participate in all evaluations conducted by Rehabilitation Services.
- Assist in the development of MHP/MHS team members as needed.
- Perform other duties as assigned.
- PLPC, PLMFT, or LMSW valid in the State of Louisiana
- A Master of Social Work degree; or
- A Master of Arts or Science degree with a major in counseling, social work, psychology or sociology.
- Ability to use computer interface to complete documentation for billing or other purposes as needed.
- Knowledge of and adept at producing effective, clinical documentation of every contact with recipient.
