PURPOSE

Under general supervision, provides routine nursing care within the limits of nursing knowledge, education, licensure, experience and ethical, legal standards of care.  Carries out all duties while maintaining compliance and confidentiality and promoting the mission and philosophy of the organization.

ESSENTIAL FUNCTIONS

  • Accurately and completely reports and documents status, care rendered, response to care, provider orders, contacts with patient and/or other healthcare providers.
  • Participates in the orientation of new employees.
  • Provides supportive services for patient requiring anti-coagulation or high risk medication therapy in accordance with the principles of patient and family-centered care according to the clinical standards of the service.
  • Acts as a primary resource for patients undergoing anti-coagulation care and/or on high risk medications.
  • Provides and coordinates direct care in accordance with the principles of medication safety according to the clinical standards of the service both in face to face and tele management approaches.
  • Safely and independently administers prescribed treatments, medications, and nursing interventions to enhance the quality of care in medication use and avoid harmful side effects.
  • Follows protocol driven algorithm to ensure proper dosage adjustment.
  • Competently uses point of care (POC) technology to obtain INR results (fingerstick blood results) that determine patient’s current therapeutic range.
  • Is able to identify at risk patients and elevate issues using critical thinking.
  • Identifies and communicates individualized patient needs for safe handoffs during transitions of care.
  • Collaborates with patient/family in care planning with all disciplines.
  • Appropriately and correctly documents the patient response to interventions and progress toward outcomes.
  • Identifies and documents variances.
  • Appropriately utilizes flow sheets, progress notes, and/or electronic medical record for documentation.
  • Provides direct nursing care to patients and distributes medications and treatments under physician order according to accepted standards of care.
  • Updates and revises care plan as needed.
  • Anticipates and recognizes changes in a patient's status and need for care.
  • Determines priorities of patient care based on essential patient needs.
  • Provides on-going assessment of patient's response to services and teaching and applies interventions as appropriate.
  • Provides telephone consultation/triage to patient, family, and/or significant others according to protocol and established guidelines.
  • Continually strives to advance knowledge and practice of clinical skills in specialty area including certification as warranted.
  • Communicates with physicians to confirm, correlate, and update patient's plan of care.
  • Accurately and completely rooms patients, takes and documents vital signs, prepares patients for provider exam, and administers medications and injections.
  • Teaches and counsels patients and families concerning heath assessment and care including prevention of disease and health maintenance.
  • Monitors inventory of medical and/or office supplies and equipment, ensuring units and/or rooms are properly stocked and supplies are current and not expired. Orders supplies as needed.
  • Accurately and completely performs patient follow-up activities including laboratory results and diagnostics, appointments, referrals, medications, and call backs.
  • Able to assist in the ordering /re-ordering of prescriptions within the guidelines of the company protocol.
  • Assists/aids with various clinic tasks/duties when needed.
  • Adheres to all company policies, including but not limited to, OSHA, HIPAA, compliance and Code of Conduct.
  • Regular and dependable attendance.
  • Follows the core competencies set forth by the Company, which are available for review on CMSweb.
  • Works holiday shift(s) as required by Company policy.
  • Responds to Emergent situations when appropriate

  • Performs other duties as assigned.

QUALIFICATIONS

Education and Experience

Required: High school diploma or equivalent.  Graduation from an accredited school of nursing. One (1) or more years of nursing (RN or LVN) experience.

Preferred: One (1) or more years of experience in related area. Registered Nurse with cardiology or anti-coagulation experience. Previous experience in an ambulatory or acute care setting.

Certificate/License: Current licensure as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in the State of Texas and current AHA approved Basic Life Support (BLS) Healthcare Provider Cardiopulmonary Resuscitation (CPR) course completion card.  Will be required to maintain a current vocational nursing license and CPR certification during employment.

Work Schedule: Monday-Friday 8am-5pm. This position will require travel to the Kyle and Southwest clinic locations.
