Job Description
- In this role, you will be part of a growing team working to improve our patient's healthcare experience.
- You will be representing PrimoCare as you develop a relationship of trust with our patients and facility partners.
- Outreach to our patients who are starting remote patient monitoring programs.
- Serve as the point of contact for patients enrolled with PrimoCare by their health care professional.
- Help patients complete assessments on their Ipads
- Engage regularly with all assigned patients to encourage consistency with the treatment plan and ensure they remain within the date range set by their provider.
- Schedule regular monthly check-ins with the patients.
- Monitor patients' data.
- Educate patients through shared information.
- Attend meetings with the PrimoCare team for information updates.
Minimum Qualifications:
- Possesses a valid CNA or Medical Assistant License + 2 years minimum of experience.
- Exhibits a strong sense of ambition, self-motivation, and self-discipline.
- Must be an excellent communicator.
- Knowledgeable about the basics of chronic health conditions related to the devices and the operation of those devices: i.e. glucose monitor, blood pressure cuff, pulse oximeter, spirometer, scale, and more.
- Proficient with technology.
- Comfortable with navigating computer platforms and familiar with smartphone functionality.
- Comfortable with patient outreach and engagement.
- Experienced with patient and Clinical staff interaction.
- Excellent communicator (written and verbal).
- Highly organized and efficient.
- Detail-oriented and good at multitasking.
- Team Player.
Preferred Additional Qualifications:
- Telemarketing/ Sales or customer service experience.
- People-oriented
- Strong clinical and technology experience.
- Patient Education experience.
Expectations and Compensation:
- Monday-Friday Hours between 9:00 AM -5:30 PM PST.
- Will receive training.
- Will report to the Care Team Manager.
