Schedule Details: Day shift 0800-1630. Holiday rotation: 2 holidays/year.

ProHealth Care offers quality in-home hospice care and in-home skilled nursing care, as well as a 15-bed residential, free standing, inpatient hospice. Our highly skilled team provides a wide array of services, including medication education and management, injections and intravenous therapy, wound care, nutritional counseling, bowel and bladder management and much more. Physical, occupational and speech therapists address home safety concerns, strengthening and rehabilitation needs. Our licensed social workers offer assistance with community resources, financial assistance and supportive care and counseling to patients and families.
Works collaboratively with patients, nurses, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary care for a patient in a home care/home hospice environment. Responsible for improving the health of patients dwelling in their own homes. This requires an individual who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. Assesses the needs of the patient and coordinates the care with an interdisciplinary team. Provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time. Experience in Home Health preferred.
Ideal candidate possesses excellent interpersonal communication skills. Strong analytical and PC skills. Experience with pre-acute and post-acute care as well as community resources. Ability to work independently and collaboratively with care providers and business office staff is necessary. Promotes safety, cost efficiency and a commitment to quality improvement.

The qualified candidate will possess the following:
- Current and valid State of WI CNA license or ability to obtain within 90 days of hire
- Experience in Home Health preferred
- EPIC proficiency desirable
- Current and valid driver's license and proof of insurance coverage as required by ProHealth.
