About the Home Health/Hospice Team
Our home health/hospice team prides itself on providing top quality care to our patients in their own home following hospital discharge, change in health condition, or transition to hospice services.
Mission for Position
Intake patients for Home Care/Hospice with compassion and high quality care.
Duties & Responsibilities

• Thorough review, acceptance or denial of incoming Home Care/Hospice referrals
• Obtain appropriate physician orders, as required by CMS
• Maintaining case manager assignments based upon geographical location
• Detailed record keeping of incoming referral information
• Other clerical duties, as assigned.

Position Type
Monday Friday 8am 4:30pm, no call rotation or weekend work, pay based upon experience
Qualifications
Qualifications

• Current LPN or MA licensure in the State of Minnesota

• Can make decisions with limited information
• Manages patient-family dynamics with ease
