Job description

Colonial Behavioral Health provides opportunities for recovery to individuals and their family members in the areas of mental illness, substance use disorder, and developmental disabilities. Colonial Behavioral Health serves the citizens of James City County, City of Poquoson, City of Williamsburg, and York County.

We are currently recruiting for a Clinician V positions within our Integrated Care team. The position will provide behavioral health consultation to children, adolescents, adults, and families at Olde Towne Medical and Dental Center (OTMDC) to improve psychosocial functioning.

The Clinician V aims to improve linkage and retention in Behavioral Health care and provide consultation and assessment within the primary care setting. To achieve this, the Clinician V monitors and tracks the overall progress of patients receiving Behavioral Health services, provides education and resource linkage to Behavioral Health patients and community partners, and provides direct clinical services to patients needing solution-focused and resource-based interventions. The Clinician V works collaboratively with internal and external partners to connect patients to services and resources needed to support their recovery and wellness goals.

This is a grant funded position.

Job Responsibilities

  • Provide comprehensive assessment and diagnosis of behavioral health clients.
  • Provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders.
  • Provide effective treatment planning and assist clients in successful referrals for ongoing long-term intervention if needed. Monitors timely initiation and the regular updating of treatment plans.
  • See patients for a limited time and a limited number of visits as needed.
  • Evaluate crisis situations and apply appropriate interventions.
  • Actively participate in meetings that support the CBH and OTMDC integrated healthcare model to provide comprehensive care for clients.
  • Assist in detecting “at risk” patients and developing plans to prevent further psychological or physical deterioration.
  • Teach patients, families, and staff care, prevention, and treatment enhancement techniques. Collaborate with OTMDC Educational Specialist.
  • Maintain a schedule of a limited number of repeat patients while also being available same day for warm handoffs from providers.
  • Monitor the CBH and OTMDC integrated health care model, identifying problems related to patient services and making recommendations for improvement.
  • Reports summary data to Integrated Care team at least quarterly and leads discussion of analysis.
  • Other duties as assigned

  • At least one year of work experience with individuals with serious mental health and/or developmental disabilities required.
  • Valid driver’s license is required.
  • Master’s Degree in a human services field required.
  • Must hold a valid license from the Board of Counseling ( LPC, LCSW, LCP, LMFT, or LSATP)_ required._

  • LPC, LCSW, LMFT, LCP certification (Preferred)
  • Certified Substance Abuse Counselor (Preferred)
