Clinician II (DH-OP) - Remote Position.

JOB SUMMARY: This position is responsible for the coordination and delivery of clinical behavioral health services designed to meet the wellness and recovery needs of the consumer. Depending on program objectives, these services may include resource development, assessments and counseling provided individually or in a group setting and may include the consumers family members.

This position is 100% remote/telehealth.

ESSENTIAL FUNCTIONS

Intake:

• Conduct initial and ongoing diagnostic assessments involving the collection, analysis, and recording of significant bio-psychosocial information.
• Formulate a diagnosis and develop a treatment plan focusing on issues mutually identified by consumer and clinician.

Clinical Services:

• Provide individual, family and group counseling to consumers as needed.
• Make collateral contacts as necessary.
• Maintain communication with clients, including outreach for hard to engage clients.
• Develop formal aftercare plans for clients completing a particular stage of treatment.
• Prepare a discharge summary for consumers whose case is being closed.

Documentation:

• Clinical documentation should be maintained in Anasazi, the agencys electronic health record software, and should include: the assessment, diagnosis form, treatment plan, progress notes, collateral contacts, and termination summary.

Case Management:

• Case management services are to be provided as needed and may include preparation of reports for external entities, community linkages, home based counseling, assistance in obtaining benefits and entitlements, referral and other client advocacy services.

Productivity:

• Clinicians are expected to spend 65% of their time in face to face consumer contact and to provide routine productivity reports to their supervisor.
• All schedules, statistical reports/logs, and clinical reports are to be submitted within proscribed timelines.
• Documentation should be completed in Anasazi within 24 hours of service delivery.

Supervision:

• Clinicians are required to participate in individual and peer supervision according to their program needs.
• Treatment plans may need to be done in consultation with the program psychiatrist.

OTHER DUTIES

• Participate in the Quality Improvement process and specific quality initiatives for the program.
• Interface with medical, vocational, residential and other program resources as necessary.
• Work as part of a team to provide services to consumers outside of primary caseload as needed.
• Supervise interns fulfilling field work requirements as needed.
• Utilize supervision and training as opportunities for enhancing professional competence.
• Performs all duties in a professional and ethical manner consistent with agency, licensing, COA, andprofessional standards.
• Other related duties as assigned.
