Form of workSt. Mary's Medical Center is seeking a LPN - Chronic Care Management to work at our HIMG- Primary Care Office. The Chronic Care LPN will work collaboratively as part of a care team to address patients’ needs and create a patient -centered plan of care. Under the direction of the provider, CCM will monitor the health and well-being of patients using certified health technology. Through various forms of communication, develop a comprehensive care plan designed to promote preventive care, address barriers to care, and support self-management of chronic diseases based on clinical standards of care. Collaborates and facilitates care based on identified needs of the patient.
Requirements
Education Requirements:
Graduate from an approved School of Practical Nursing. Currently licensed to practice nursing in the State of West Virginia.
Experience:
A minimum of two (2) years of experience in a clinical and/or case management nursing preferred.
Certifications/Skills:
Position requires strong verbal and written communication skills. Must be compassionate, able to build strong relationships with patients and other caregivers, work collaboratively with others, solve problems and be flexible and technology proficient. BLS required.
