Responsibilities
Responsible for the overall administration and clinical supervision of the department's Health Homes Care Management Program. Lead staff in the delivery of services in a "Person Centered" manner.
Work in collaboration with the HOPE Center's medical and support service staff as well as the hospital's inpatient discharge planning, emergency department and behavioral health services to ensure member's seamless transition of care and on-going engagement with providers
1. Maintain an understanding of clinical issues and appropriate clinical interventions to address these issues, including evidence-based interventions for the populations identified in the program(s). Responsible for overall operation of the assigned program(s).
2. Provide excellent and high-quality care to all internal and external customers, including clients and staff.
3. Oversee compliance with documentation and billing requirements for Departmental programs to assure an optimum & ongoing revenue stream for the services.
4. Coordinate the program's response to the data requirements & narrative reporting requirements of the various funding sources by insuring all required statistical reports and case notes are done by the 5th of each month.
5. Evaluates and improve overall support services at HOPE Center through participation in Department's Quality Improvement efforts. This includes participation in yearly performance improvement activities for programs supervised by this position.
6. Obtains required education and other requirement and maintain all required licensure at the level of LCSW.
7. Ensure that all tasks and communications are completed and responded to within the allotted time frame.
8. Provide effective customer service as well as culturally and linguistically appropriate care to all customers (internal and external), clients, and patients.
9. Develops and manage the client input mechanisms including client satisfaction surveys and development and implementation of a Consumer Advisory Committee, if required for program.
10. Provide Supervisory coverage for HOPE Center during Saturday clinic hours, on a rotating basis with the rest of the Department's Management Team.
11. Other job duties as requested by supervisor.
Additional Duties - Health HomesClinical Supervisor
1. Oversee the operations of the HH Care Management program. Ensuring compliance of documentation and billing requirements for the HH Care Management program. Remain current on NYS - DOH and Lead Health Homes policies and procedures. Ensure the programs policies and procedures reflect the changing requirements.
2. Coordinate the program's response to the data requirements.
a. Monitor the Care Management platforms to ensure.
i. Case notes meet the program requirements and are written within one business day of the intervention.
ii. Service Plan (Care Plan) elements are current and appropriate.
iii. Assessments and Reassessments are completed as per HH policy and needs are identified in the client's Service Plan
iv. HCBS requirements are being met.
v. Roster management, assignment and caseload disposition are done in the required time frames.
vi. Transitions of care requirements are met as per HH and DOH policy.
vii. Case conferences happen quarterly with health providers as per DOH/AI policies.
b. Remain current on the Lead Health Homes quality indicators and ensure program compliance.
3. Responsible for overall operation of the Health Homes Care Management program including:
a. Maintaining positive working relationships with coordinating and referring agencies and program staff to assure positive program outcomes. Coordinate and process all program referrals and ensure minimum caseload size.
b. Ongoing, proactive, and effective communication with clients, staff, and other Departments in hospital to assure the highest level of client/patient care.
c. Regular proactive and effective participation in meetings with the Lead HH (HVC).
d. Perform management activities including interviewing, hiring, evaluation, and termination of assigned program staff. This also includes scheduling staff time off and ensuring adequate staffing coverage to provide safe and effective client care.
e. Direct and supervise the assigned staff, promptly addressing all program challenges in a pro-active and solution-oriented manner.
f. Provide individual clinically focused supervision to all assigned staff.
g. Supervises in the field and observes staff working to provide supervision on observed behaviors.
h. Assure assigned program's viability by ensuring that monthly, quarterly, and annual measurement indicators are met at or above the required rate.
i. Assure that all Health Homes deliverables are met, and that high quality patient care is performed.
j. Conduct annual audits as required by the Lead HH and/or DOH.
k. Provide "After hours" coverage via a 24/7 Call Service as needed.
4. Work in conjunction with the Department's Head Nurse and other Clinical Supervisors to assure an interdisciplinary team approach is utilized in all patient care. Work with the Head Nurse to assure that all client clinical care is coordinated.
Qualifications
  • NY State LCSW license required
  • LMSW will be accepted with the following conditions: 1) working towards LCSW licensure 2) will have a split of 80% Administrative / 20% Direct service provision (will carry a caseload), 3) will receive clinical supervision from a LCSW.
  • Five years supervisory experience with a focus on supervising Care Management services and direct line Care Managers in HIV, HIV prevention, Care Management, alcohol and/or substance abuse programs.
  • Three to five years' experience treating co-morbid disorders (mental health and substance abuse).
  • Basic computer knowledge.
  • Ability to effectively communicate in English, both verbally and in writing.
  • Excellent documentation skills required.
  • Strong interpersonal skills.
  • Valid driver's license required.
  • Access to personal car & able to independently travel between multiple off-site locations required.

Preferred:
Additional languages preferred; strongly prefer bilingual/bicultural Spanish/English and will consider Spanish/Creole.
