We are small family medicine practice seeking a skilled and compassionate Registered Dietitian/Nutritionist to join our team. You will be responsible for providing nutritional counseling and guidance to patients to help them achieve their health goals. You will work closely with the healthcare team to develop personalized meal plans and educate patients on proper nutrition. The ideal candidate will have a strong background in health coaching and patient assessment.
RESPONSIBILITIES
- Conduct patient assessments to determine nutritional needs and develop individualized meal plans
- Provide nutrition counseling and education to patients on healthy eating habits and lifestyle modifications
- Monitor patient progress and make adjustments to meal plans as needed
- Collaborate with healthcare professionals to ensure coordinated care for patients
- Stay up-to-date on the latest research and trends in nutrition and dietetics
- Document patient interactions and maintain accurate records
