In consultation with the Nephrology Medical Director, the Clinical Program Specialist manages behavioral/mental health projects delegated by the Program Director. The Clinical Program Specialist provides support and consultation for all Division of Nephrology patients. Primary responsibilities are to the support of transplant and dialysis patients in compliance with all CMS regulations.
ROLE RESPONSIBILITIES
ROLE RESPONSIBILITIES
Responsible for all behavioral/mental health innovations, programs/projects/pilots for the Division of Nephrology
Collaborates with Medical Director to determine project charter
Follows project management processes: define scope, review deliverables, create process, and generate timeline
Identifies challenges and barriers and develops solutions
Completes project work effectively, efficiency, and within specific time frames
Responsible for data collection and producing metrics for reporting purposes
Plans, develops, and implements strategies for identifying and generating funding and/or resource opportunities for behavioral/mental health program activities, to include development of grant and other fundraising proposals.
Coordinates behavioral/mental health Community Outreach and advocacy as appropriate
Engages in community, state, national relationship building and sustainability
Facilitates and/or participates in regional and state collaboratives, work groups, coalitions. Generates discussion, interest, policy, and advocacy for behavioral/mental health issues and policies
After hours and weekend on call coverage for the Division of Nephrology
Position Specific Role Responsibilities
Qualified licensed clinical social workers with Master of Social Work degrees are required by the Centers for Medicare Medicaid Services to be part of the transplant center's multidisciplinary team. Social workers complete comprehensive psychosocial evaluations for all patients referred for transplant or live organ donation to determine if they meet psychosocial selection criteria.
The psychosocial patient selection criteria and psychosocial evaluation address psychosocial issues that affect patient and graft survival outcomes.
Education and Experience
Masters' degree in Social Work with a LCSW (or with the expectation of attaining LCSW in 1 year). Progressively increasing experiences as a social worker related to the responsibilities describes above.
Experience with transplant and dialysis patients preferred.
