***Per Diem employees must make themselves available to work a minimum of 3 full shifts per month in their home department, one of which is a weekend shift in departments with weekend staffing. Availability must be provided to the department prior to the closure of the department's scheduling process. Availability must align with open shifts on the schedule. Must be available to work 1 premium pay summer holiday and 1 premium pay winter holiday. In addition to advanced scheduling, the Per Diem employee may be called to help with last minute scheduling needs***
Summary of Position:
Ensures that the pharmacy provides optimal clinical services on a daily basis, promoting desirable patient outcomes through safe and cost-effective drug therapies. Establishes and maintains productive collaborative relationships with practitioners and hospital staff. Supports all safety and regulatory initiatives undertaken by the hospital and department. Demonstrates the knowledge and skill necessary to identify and provide pharmaceutical care to the populations served at the Barton Healthcare System: including neonatal, pediatric, adult, geriatric, obstetric, chemically dependent, and rehabilitation patients. Participates in all aspects of hospital pharmacy services including but not limited to operations, distribution and clinical pharmacy activities.
Qualifications
Education:
• Pharm.D. required
• Prefer completion of PGY 1 or 2 clinical residency
Experience:
• Minimum 1 year experience in hospital/similar acute care pharmacy setting
• Prefer completion of residency or equivalent experience
Knowledge/Skills/Abilities:
• Relevant clinical pharmaceutical knowledge
• Strong interpersonal communication skills
• Computer literacy in common Microsoft applications
• Knowledge of electronic medical record or equivalent
Certifications/Licensure:
• Valid and clear California pharmacist license
• Prefer valid and clear Nevada pharmacist license
• Current BLS certification
• Prefer ACLS and PALS certifications or ability to obtain

Essential Functions
1. Provides consistently exceptional care at all times.
2. Demonstrates the knowledge and skills necessary to identify and provide pharmaceutical care to the populations served at the Barton Healthcare System: including neonatal, pediatric, adult, geriatric, obstetric, chemically dependent, and rehabilitation patients.
3. Ensures that the pharmacy provides optimal clinical services on a daily basis, promoting desirable patient outcomes through safe and appropriate drug therapies.
a. Monitors drug therapy of all admitted and infusion center patients
b. Collaborates with providers to both optimize and address any issues with drug therapies, including but not limited to:
i. Appropriateness of drug and dose
ii. Drug interactions and contraindications
iii. Adverse reactions
iv. Renal dose adjustments
v. IV to PO conversions
vi. Therapeutic substitutions
vii. Antibiotic appropriateness as part of the antimicrobial stewardship program
c. Provides pharmacokinetic dosing and consultation for provider specified medication therapies according to hospital protocols and proceduresProvides clinical consultation for anticoagulation, pain management, opioid conversion, and multimodal pain management as indicated
d. Provides clinical consultation and clarification to practitioners as indicated, addressing IV compatibility issues, medication timing, and providing other medication information
e. Participates in multidisciplinary rounding with the health care team
f. Performs patient counseling
i. For inpatients on new therapies or discharge medications as indicated
ii. For infusion center outpatients as indicated for new therapies or to address questions during on-going therapy
g. Participates in medication reviews including the addition and removal of medications to the hospital formulary and participates in drug use evaluations
h. Participates in full traumas and code blues to provide pharmaceutical support and expertise
i. Facilitates the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved and authorized by the appropriate medical staff committees
j. Participates in departmental and hospital-wide committees and task forces, modeling excellence and professionalism in communications.
4. Ensures that the pharmacy provides optimal distribution services on a daily basis, promoting safe and efficient medication delivery
a. Responsible for the safe and accurate distribution of drugs to inpatients, outpatients, and clinics
b. Responsible for ensuring all compounding activities are performed safely and efficiently
c. Responsible for overseeing the daily operations and for ensuring all relevant laws, policies and procedures are followed appropriately by pharmacy staff members
d. Supervises and directs pharmacy support personnel in a professional manner.
i. Verifies and oversees the daily activities of the pharmacy technicians as required for the operations of the pharmacy.
e. Mentors new pharmacists, pharmacy technicians and pharmacy interns
5. Establishes and maintains productive collaborative relationships with practitioners and hospital staff
6. Supports all safety and regulatory initiatives undertaken by the hospital and department
7. Completes annual mandatory education related to clinical pharmacy practice and compounding practices in a timely manner
8. Performs monthly area inspections and collaborates with staff to ensure compliance with all relevant hospital policies, best practices and laws
9. Performs monthly clinic inspections for assigned areas ensuring pharmaceutical compliance at each site with all relevant laws and regulations
10. Provides on call pharmaceutical support for clinically necessary situations when assigned
11. Identifies and collaborates to correct EMR build issues related to medications. Performs ongoing testing and build validation in conjunction with the Information Technology department.
12. The Clinical Pharmacist is under the supervision of and reports to the Director of Pharmacy.
13. Responds to the needs of the department by performing other duties, as necessary.
