Location
This is a predominantly remote role, with the expectation to come onsite at Assembly Row in Somerville for routine team meetings and limited in-person provider meetings. Potential geographic regions include the North Shore, Metrowest, and Milford.
General Overview
The Clinical Pharmacist will be responsible for optimizing patient's health outcomes by providing the highest level of patient care for a high-risk patient population. In this role, the Clinical Pharmacist will apply subject matter expertise in managing the pharmacy component for high-risk patient care management, as part of a Care Management team, focusing on comprehensive medication management reviews, therapeutic alternatives, medication adherence, post-discharge medication reconciliation, and recommending cost-effective drug therapy regimens. Will also be responsible for providing primary care-focused pharmacotherapy services and evaluate and manage patients with chronic conditions (e.g., Type 2 diabetes, hyperlipidemia, hypertension, COPD/Asthma, heart failure, smoking cessation, ESRD) through direct patient care. Additionally, the Clinical Pharmacist will serve as a resource and drug expert for all primary care-related pharmacy questions as well as address ambulatory pharmacy costs and trends through the implementation and management of pharmacy utilization activities.
Responsibilities
  • Collaborate with the Care Management team and other health care providers to provide pharmacist support to a high-risk patient population. This requires direct patient involvement through phone calls or office visits.
  • Provide drug education and counseling to patients by utilizing motivational interviewing to promote behavior change.
  • Perform comprehensive medication management (CMM) review for all newly enrolled Integrated Care Management program (iCMP) patients and when new diagnosis/high-risk medication is added. Otherwise, CMM performed on a yearly basis for all iCMP patients.
  • Perform transitions of care medication reconciliation when a patient is discharged from home from hospital or skilled nursing facility as part of post-discharge assessment.
  • Assess compliance issues and develop plan to improve adherence.
  • Address affordability concerns, including, but not limited to cost-effective drug therapy regimen, simplification of medication regimen, and prescription assistance programs.
  • Assist iCMP team in identifying medication issues that impact readmissions and develop strategies to improve post-discharge outcomes.
  • Develop, document, and execute a patient-centered plan of care utilizing national guidelines or established local protocol.
  • Accountable to iCMP program key performance indicators as appropriate.
  • Document patient encounters, update medication lists, and communicate with providers and iCMP care team members vie Epic or other electronic patient record.
  • Take an active role in the development and implementation of quality improvement projects.
  • Participate in local site activities, including team meetings, curbsides with care team members and physicians, provider meetings, etc.
  • Serve as a therapeutic resource to assigned medical groups.
  • Maintain own professional and specialized competency through continued education.
  • Collaborate with colleagues on other job-related duties and projects as directed.

Qualifications
Qualifications
  • Must be a graduate of an accredited college or university with a Pharm D degree.
  • Board certification is required.
  • Licensed to practice pharmacy in the Commonwealth of Massachusetts required.
  • Experience functioning as a Clinical Pharmacist in ambulatory setting preferred.
  • Experience in managed care setting preferred.
  • At least three years of related experience preferred.
