Part time position to support the clinical pharmacy team. Seeking Clinical Pharmacist to provide clinical pharmacy services to support medication management at East Boston Neighborhood Health center including drug information support, patient reviews, medication therapy management services, provider support, and to support the overall clinical pharmacy department and initiatives.Responsibilities Include:
Patient Care
- Assess and monitor the drug therapy needs of patients through consultations, patient education and clinical laboratory monitoring. Assess patients for medication adherence barriers and develop a plan to maximize patient’s medication success through improving or maintaining adherence to medications.
- Based on this assessment and in conjunction with the patient, the patient’s physician and other members of the team, create a care plan that will address the identified needs, remove barriers and improve the health of the patient.
- Provide patients with a personal medication record that can be shared with caregivers, with prescribers, and across care transitions.
Provider Support
- Develop and deliver physician drug education plans & strategies (including pharmacy-specific benchmark reports) to promote cost-effective prescribing
- Serve as expert in clinical pharmacokinetics & pharmacodynamics and is knowledgeable in drug information including contraindications and dose adjustments
Care Management Collaboration
- Serve as an accessible authority on drug usage, interactions, over-dosages, compliance and other pharmaceutical information including non-formulary and restricted drug determinations
- Develop and deliver pharmaceutical education to health team
- Assist in development and execution of pharmacy-specific population health strategic and tactical plans including HEDIS measure and CMS Star rating improvement
- Function as liaison and subject matter expert related to client PBM systems and processes most specifically related to drug utilization management, drug prior authorization and pharmacy claim adjudication
Pharmacy Program Support
- Assist in development and lead the execution of clinical pharmacy programs & initiatives and other Pharmacy and Therapeutics activities
- May serve as preceptor for pharmacy students and pharmacy residents in accordance with established protocols
- Guide and report on 340B processes and utilization
- Serve as a liaison between the dispensing pharmacy and clinical activities Health Center
Qualifications:
Required
- Pharmacy doctorate degree
- Current active pharmacy license in good standing in Massachusetts
- Working knowledge of 340B program
- Exceptional interpersonal, presentation, oral and written communication skills
- Self-driven with the ability to recognize areas of potential program growth
Preferred
- Completion of 1 year residency related to practice area or 3 years of equivalent clinical practice experience
- Board certification (Pharmacotherapy (US-BCPS) or Ambulatory Care (US-BCACP)) – a plus
- Certified patient educator (e.g., Diabetes, Asthma)- preferred
- MTM certification- a plus
- Health plan or PBM clinical pharmacy experience (1-2 years)
