- Work with patients, PCPs, pharmacies, and health plans to achieve 4+ STAR performance in pharmacy related HEDIS measures (statin use in patients with diabetes and cardiovascular disease, medication adherence for management of diabetes, cholesterol, and hypertension)
- Complete comprehensive medication reviews and collaborate with PCPs in support of optimal medication therapy for highest risk patients
- Provide post discharge medication reconciliation for high risk cases
- Participate in clinical rounds to support patient care planning
- Provide report outs and recommendations on STARs performance related to pharmacy measures for the Massachusetts market
- Develop clinical pharmacy strategies and support the dissemination of clinical information (e.g., provider talking points, patient education, industry updates, etc.)
- Deliver training to staff and providers in support of quality medication management and STARs/HEDIS performance
- Vaccine administration support in the clinic as needed
- Development and direct responsibility for refill program and operations
- Analysis of available RPM and clinic data to enable deprescribing when indicated
- Development of patient educational programs and documents to describe medications and disease treatment programs.
- Direct patient consultation in collaboration with PCPs to develop effective drug treatment strategy
- Active pharmacist license in the state of Massachusetts
- PharmD or other advanced pharmacy degree
- 2+ years as a practicing pharmacist, ideally in an ambulatory care practice setting with responsibility for STARs pharmacy metric performance
- Knowledge of PBM and health plan review process
- Knowledge of CMS regulations and HEDIS/STARs requirements
- Vaccine administration certification
