Clinical Liaison (Rn Or Lmsw)- Queens

The Hospice Liaison will interact and influence the referral patterns of healthcare professionals in hospitals, nursing homes and private practice. Your success will be derived from the provider relationships that result in referrals of appropriate patients from physicians, discharge planners, case managers, floor nurses and other healthcare professionals. You will also provide clinical expertise, medical communication and patient/family education for the successful referral of patients.
Requirements:

• Current NYS/RN or LMSW license.
• 2+ years of RN or LMSW experience
• Bilingual, Spanish-speaking preferred
• Previous experience in Hospice, Homecare, Healthcare sales or Marketing.
• Exceptional interpersonal and communication skills.Must be an effective self starter with superior problem solving skills.
• Level of proficiency in Microsoft Word applications and EMR applications.
• The desired candidate must be willing to learn Medicare regulations and related reimbursement issues for hospice and long-term care.
• Must have a reliable automobile and valid drivers license.

Job Type: Full-time

Pay: $90,000.00 - $105,000.00 per year
