This position will be assigned to our Brooklyn service area.
Seeking dynamic Registered Nurse with exceptional interpersonal skills and a passion for excellent care to join our Provider Relations Team.
Responsibilities: Main responsibilities are to act as a patient advocate promoting effective transitions between facilities and hospice services. Act as a primary liaison between physicians, hospitals and other healthcare providers. Provide clinical expertise, medical communication and patient/family education for the successful referral of patients.
Qualifications:
- Current NYS RN license
- 3+ years of RN experience
- Exceptional interpersonal and communication skills
- Ability to work with a diverse interdisciplinary team
- Proficiency in Microsoft Word applications and EMR (electronic medical records)
- Hospice or Homecare experience a definite plus
- Marketing or Sales a definite plus
- Valid Drivers License
- Reliable motor vehicle
- Bilingual, English/Spanish a plus
