Clinical Coordinator/ Social Worker

Hours: 40 hours per week
Primary Objective:
The Veterans Clinical Coordinator plays a vital role in ensuring the provision of high-quality clinical care and support to veterans. This position involves coordinating and managing the clinical services and resources for veterans, while also providing case management and coordination of care. The primary objective of the Veterans Clinical Coordinator is to enhance the overall well-being and healthcare experience of veterans. The expectation is that this position will be in-person, with the possibility of occasional remote work as necessary.
Clinical Responsibilities:
· The Veterans Clinical Coordinator will work closely with veterans, their families, healthcare providers, and community resources to ensure the delivery of comprehensive and coordinated care.
· This position requires a strong understanding of the unique healthcare needs and challenges faced by veterans, including physical, mental, and emotional health issues.
Job Duties:
· Case Management: Provide comprehensive case management services to veterans, including assessment, planning, coordination, and monitoring of their healthcare needs.
· Clinical Coordination: Coordinate and facilitate communication between healthcare providers, veterans, and their families to ensure continuity and quality of care.
· Resource Management: Identify and connect veterans with appropriate community resources and support services, such as housing assistance, mental health counseling, vocational training, and financial aid.
· Treatment Planning: Collaborate with the veteran and other program staff to develop personalized treatment plans for veterans, ensuring that their specific needs and goals are addressed.
· Advocacy: Serve as an advocate for veterans, ensuring their rights and preferences are respected, and help navigate the healthcare system to overcome barriers to care.
· Education and Support: Provide education and support to veterans and their families, empowering them to actively participate in their healthcare decisions and self-care management.
· Documentation: Maintain accurate and up-to-date records of all clinical activities, assessments, interventions, and outcomes, adhering to confidentiality and privacy regulations.
Qualifications:
· Education: bachelor’s degree in nursing, social work, psychology, or a related field. Associate’s degree in a similar field plus 4 years of experience. Alternatively, Individuals with 6 years of work experience may also be considered.
· Licensure: None required. Licensing of Alcohol and Drug Counselor, Bachelors/master’s level Social Worker, RN, LPN, CNA are preferred.
· Experience: Minimum of 2 years of experience in clinical coordination, case management, or a related role, preferably within the veteran healthcare system.
· Knowledge: In-depth understanding of the healthcare needs and challenges faced by veterans, including knowledge of military culture, post-traumatic stress disorder (PTSD), traumatic brain injury (TBI), and other common veteran health issues.
· Communication Skills: Excellent verbal and written communication skills to effectively collaborate with veterans, their families, healthcare providers, and community resources.
· Organizational Skills: Strong organizational and time management skills to prioritize tasks, manage multiple cases, and ensure timely coordination of care.
· Compassion and Empathy: Demonstrated ability to provide compassionate and empathetic care to veterans, respecting their unique backgrounds, experiences, and healthcare preferences.
· Problem-Solving Skills: Effective problem-solving and critical-thinking abilities to assess complex situations and develop appropriate care plans.
· Computer Skills: Proficiency in using electronic medical records (EMR) systems and other relevant software applications.
