POSITION TITLE: Clinical Coordinator I – Solutions to Violence
JOB TYPE: Regular, Full-Time, Exempt, Union, Benefitted
REPORTING RELATIONSHIP: Director and/or Designee
SUMMARY:
Under general supervision of the Director or their designee, the Clinical Coordinator I – Solutions to Violence (STV) provides trauma-informed and survivor-centered clinical consultation and direction of client care and referrals to the staff, develops, and maintains competent, quality client services and well-coordinated community education efforts. The Clinical Coordinator I collaborate with public and community-based agencies, as needed, to ensure quality service delivery. Services are provided throughout Santa Clara and San Benito Counties.

Education and Experience:
Required:
  • Master’s Degree in mental health profession and a minimum of two years direct client work experience in related field.
  • Experience working with survivors of trauma, abuse, and/or exploitation, or individuals experiencing mental health challenges. 
  • In collaboration with various County services, experience working with systems to meet requirements of the Agency and contracts.
Preferred:
  • The individual will preferably be bilingual and bicultural.
Knowledge:
Required:
  • Clinical knowledge and expertise within scope of practice in group, family, and individual therapy.
  • Empowerment, advocacy and support techniques for victims/survivors of violence, people with mental health disorders or substance abuse issues.
Preferred:
  • Possess understanding of the dynamics of intimate partner abuse, sexual assault, and human trafficking, and the root causes of violence against women.
  • Working knowledge of wellness and recovery concepts.
  • Knowledge of multiple partner systems, case management, crisis intervention, and resources and referrals.
  • Working knowledge of Evidence Based Practices.
  • Working knowledge of outcome driven care.
Special Requirements: Must be able to receive a criminal records clearance as required by specific programs, licensing regulations and Community Solutions practices. Completion of 75-hours Intimate Partner Abuse/Sexual Assault/Human Trafficking (IPA/SA/HT) training upon hire or within 6 months of employment.   Must be able to obtain and remain qualified for County 5150 Hold authorization. Position requires an automobile to perform routine job duties.
License: Must be registered and in good standing with California Board of Behavioral Science (BBS) as an Associate Marriage and Family Therapist (AMFT), Associate Clinical Social Worker (ASW), and Associate Professional Clinical Counselor (APCC). Possess a valid California driver’s license, current car insurance and a good driving record as documented by a DMV report and consistent with the Agency’s liability insurance requirements.
What You'll Be Doing in the role of Clinical Coordinator I – Solutions to Violence: The following are examples of essential job duties for this position. The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned to support the program and Agency.
  • Provides leadership and program coordination to ensure the needs of clients are met and services meet contract and/or certification requirements.
  • Coordinates staff and the program’s clinical activities and reports/updates Program Director and Coordinators on status.
  • Assigns schedules and monitors caseloads in partnership with managers, for staff, interns and volunteers.
  • Reviews charts on a weekly basis with staff to ensure audit ready charts. Supports audit practices for the program. Emails staff and copies the Director, managers and admin support on the outcome of auditing results with follow up email for any needed corrections with reports/updates to Program Director.
  • Provides Program Manager with weekly program status on meeting documentation requirements and timelines.
  • Responsible for coordination for clinical care to provide for clients’ stated needs and goals.
  • Supervises the clinical skills development of interns, trainees and waivered employees. Conducts initial orientation, training and development. Responsible for all required documentation and reporting required by the academic programs and the BBS process as assigned by Program Manager.
  • Enforces all Agency protocols and provides feedback on response forms to referral sources, follows up on pending referrals, ensures timelines are met and referrals are opened and discharged within program timelines.
  • Monitors chart documentation timelines, tracks and evaluates program outcomes and submits progress reports in a timely manner, as assigned.
  • Responds to phone calls and emails within 24 hours on business days unless extenuating circumstance exist. Resolve outstanding issues collaboratively using strength-based solutions focused approach.
  • Attends and participates in Agency meetings and contributes to continuous quality improvement activities to enhance team and program development.
  • Consistently uses strength-based, trauma-informed, culturally responsive and solution focused problem-solving skills.
  • Maintains excellent customer service skills with internal and external customers.
  • Leads regular meetings with program staff and/or volunteers in order to train, schedule services, plan, anticipate and overcome obstacles and share information as assigned.
  • Attends all County/community meetings as assigned and shares pertinent information with the team.
  • Provides direct services, as needed. Provides back-up crisis response coverage for evening and after hours.
  • Responds to crisis situations and assists staff, clients and their families with problem resolution.
  • Performs involuntary hospitalization assessments and arranges for hospitalization, if necessary, may serve as an expert witness in court cases.
  • Conducts screening, intake and assessments and carries a caseload, as assigned. Assesses clients’ needs and goals to determine appropriate resources, including the referral to professional services without delay.
  • Completes detailed clinical records and other required documentation in accordance with agency, program and regulatory requirements. Reviews and signs off on clinical documentation within Agency timelines for paraprofessional staff, and other staff, as assigned.
  • Reinforces current policies/procedures and recommends changes/improvements as needed.
  • Provides education, training and presentations to staff, volunteers, clients, families and the community.
  • Develops and maintains collaborative relationships with appropriate community organizations and system providers.
  • Successfully completes all training requirements for this position to ensure agency and contract compliance.
  • Frequent travel required.
  • Employee must have the ability to operate their personal or Agency motor vehicle to participate in client visits, provide daily transportation of clients and/or other assigned Agency business.
  • May participate in on-call and after-hours rotation, as needed. Flexible schedule may be required to work some evenings and weekends.
  • Performs first aid as needed based on training and certification.

Physical Requirements:

Sit for extended periods; frequently stand and walk; normal manual dexterity and eye-hand coordination; lift and move objects weighing up to 25 lbs.; corrected hearing and vision to normal range; verbal communication; use of office equipment, including computer, telephone, calculator, copiers, and FAX.
Work Environment and Location: This position is located within commutable distance to our Gilroy headquarters or San Jose office and is open to a hybrid work schedule, with occasional needs to be able to travel into our offices and work in the evenings or weekends.
