The Case Management position is responsible for providing quality care to children & Adolescents, and their families and guardians throughout the patients’ episode of care. Quality care includes, but is not limited to the following: performing psychosocial assessments, participating in multi-disciplinary rounds and treatment planning meetings, providing care coordination, organizing individual/family/systems meetings, facilitating clinical process groups, managing the discharge and continued care planning process and documenting all patient encounters.
This position qualifies for the MA loan Repayment Program
-Review of patient's clinical documentation from referring facilities
-Conduct individual sessions with all clients minimally one-time weekly
-Complete timely and thorough documentation:
- Psychosocial Assessment
- Outreach and care coordination with patient's providers
- Initial Multidisciplinary Treatment Plan
- Treatment Planning updates
- Progress notes
- Discharge Plan
-Participate in weekly Case Management Departmental Meetings
-Participate in process/quality improvement measures
-Facilitate process groups
-Complete required CPI, CPR and other orientation and annual hospital trainings
-Maintain appropriate state licensure
