REPORTS TO: Home Health Administrator
POSITION SUMMARY:
Reporting to the Home Health Administrator, the Clinical Manager is responsible for the clinical operations and provides direct supervision of all clinical services personnel and coordinates care services.
EDUCATION AND QUALIFICATIONS:
- Active RN license with the State of Florida
- Minimum of two (2) years experience in home health nursing required
- Home health care and quality assurance experience preferred
- Minimum of one (1) year of supervisory experience as a registered nurse preferred
- Thorough knowledge and understanding of the operations of a home health agency and Medicare requirements and regulations required
- OASIS review experience required
- Knowledge of Home Health COPs and state and federal regulations
- Knowledge of Home Health Compare Star Ratings, Quality Outcomes, and Process Measures
- Proficient with Microsoft Office and EMR use
ESSENTIAL JOB RESPONSIBILITIES:
- Directs nursing and home health care as prescribed by the attending physician. Reviews all treatment orders for accuracy, appropriateness and completeness.
- Monitors the records maintained for all services rendered to patients. Review all clinical notes and OASIS assessments for accuracy, medical necessity and homebound status in a timely manner.
- Evaluates and regularly reevaluates the therapy and nursing needs of each patient.
- Supervises and trains other nursing service personnel. Responsible for obtaining accurate referral information.
- Obtains specific physician orders for home care services including the plan of care. Obtains face to face documentation and lab results to support skilled services requirement.
- Ensures that documentation required by the Medicare Conditions of Participation (CoPs) are generated and sent to facilities physicians, patients and powers of attorney according to the specified time span.
- Ensures that physicians orders are communicated to home health agency staff with accuracy, thoroughness and understanding.
- Communicates patients data and observations verbally and through documentation.
- Collaborates with the physician to implement changes to improve patient care.
- Reports to the physician the patients response to treatments, including drugs, and changes in the patients physical or emotional condition as needed.
- Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide care appropriate to the age of the patients served.
- Supports and carries out the policies of Gulfside Home Health.
- Participates in Gulfside Healthcare Services quality and compliance programs.
- Assures that monthly case conferences are done on each patient.
- Presents weekly report to the Home Health Administrator.
- Performs other administrative duties as assigned by the Home Health Administrator.
- Assists the Home Health Administrator in meeting the federal, state and local regulations and standards.
