Schedule:Mon - Fri 8am-4pm
Salary:$70,300 - $74,000 / year, depending on licensure
We are seeking a compassionate, caring, Clinical Care Manager to provide care coordination and Clinical Care management for One Care enrollees with complex healthcare and behavioral health needs. The Clinical Care Manager collaborates with Vinfen's Adult Clinical Community Services (ACCS)teams and One Care Plan's clinical staff to maximize coordination and integration of care. The Clinical Care Manager will be at the helm of organizing and coordinating resources and services in response to the individual healthcare needs across multiple settings.
The Clinical Care Manager will conduct initial and ongoing risk assessment, design personal crisis management plans, develop relapse prevention and harm reduction strategies, collaborate with One Care Plan, Primary Care Provider and other health care providers regarding changes in services, care transitions and crisis intervention while focusing on continuity and quality of client care as well as potential efficiencies and cost-savings.

Requirements
Required Credentials/Licenses:
  • License to practice as "Licensed Practitioner of the Healing Arts" in Massachusetts required (i.e. LCSW); MA license to practice independently (LICSW, LMHC) preferred.
  • Master's Degree in clinical counseling field required.

Additional Requirements:
  • Minimum of 5 years' clinical and case management experience preferred.
  • Effective skills in managing, teaching, negotiating, and collaborating with multidisciplinary teams and client/family focus; experience developing care plans.
  • Strong organizational and time management skills. Excellent written and oral communication skills.
  • Effective relationship management and team building skills.
  • Experience working with people with psychiatric disability, co-occurring disorders preferred.
  • Preference given to bi- lingual/bi-cultural applicants and applicants with lived experience of psychiatric conditions. In some cases, experience may be substituted for academic training.

Knowledge and Skills:
  • Strong knowledge base in clinical and financial aspects of care
  • Strong commitment to the right and ability of people served by Vinfen to live, work, have
    meaningful relationships and receive the resources and supports needed in their community ofchoice
  • Knowledge of person-centered, strengths-based, recovery-oriented values and principles andmodalities
  • Knowledge of clinical and psychiatric rehabilitation values, principles, and techniques
  • Knowledge of health risks of psychiatric disability
  • Knowledge of health promotion and Clinical Care coordination techniques
  • Knowledge of motivational interviewing, stage of change and harm reduction techniques
  • Knowledge of trauma-informed and culturally competent services
