Responsible for professional nursing care and related assistance to patients/families. The Clinic Site Nurse is involved in direct and indirect patient care. This position provides direction to Licensed Vocational Nurses (LVNs) and Medical Assistants (MAs) as well as other non-licensed nursing personnel in the provision of patient care. In collaboration with other health team members, follows the philosophy, policies and goals of LSCC.
Daily Job Duties:
- Performs patient assessments/reassessments including triaging
- Coordinates and participates in providing direct and indirect patient care
- Maintains patient care standards
- Interventions are implemented in a safe and appropriate manner and documented
- Performs care to meet the physical, psychological, emotional and social needs of the patients/families
- Care is based on the patient’s age, developmental stage, educational needs and family/guardian expectations
- Contacts the appropriate health care provider of patient/family concerns
- Manages the referral process ensuring timely and efficient referrals to specialty services and communicates barriers to the appropriate Provider
- Interprets reports, data and medical information to determine the needs of the patient/family
- Interprets an extensive variety of technical instructions and manages several abstract and concrete variables
- Provides for patient safety at all times
- All other duties as assigned
Clinic Duties:
- Collaborates with the Interdisciplinary Team in the planning of patient care
- Participates with leadership in data collection and quality improvement initiatives
- Performs clinical and non-clinical skills proficiently and in accordance with policies and procedures
- Manages the acquisition, storage and maintenance of vaccine and maintains supporting documentation and records
- Assists with monthly reporting for programs as directed by the Clinic Site Manager or Practice Administrator
- Organizes work and utilizes time efficiently based on knowledge of nursing theories and procedures
- Performs clinic specific technical skills
- Utilizes all equipment in the Clinic with maximum efficiency and effectiveness based on knowledge and education
Training:
- Teaches, supervises, and serves as a resource person and delegates in accordance with the competency level of all staff members. Serves as a mentor and completes the appropriate documentation of training and competency
- In collaboration with other members of the health team provides health care teaching to patient/family or significant other (in compliance with HIPAA Standards)
Compliance:
- Collaborates with the Clinic Site Manager and Practice Administrator in the planning, training and development of Medical Assistant core competencies
- Coordinates with Pharmacy staff to maintain compliance and documentation for formulary, sample inventory and prescription assistance programs as directed by the Site Manager and/or Practice Administrator
- Verifies accuracy of medication administration by following all safety measures
- Assumes responsibility for his/her own professional development and for sharing knowledge with others
- Invests own time and effort to improve knowledge, skills and judgment
REQUIRED EXPERIENCE, EDUCATION, AND LICENSE:
- Graduate of accredited nursing school
- Current valid State of Texas Registered Nurse (RN) license
- Basic Life Support (BLS) certification from American Heart Association or American Red Cross required
PREFERRED EXPERIENCE, EDUCATION, AND LICENSE:
- Previous work experience in outpatient/clinic nursing
- Previous work experience using Electronic Medical Records (EMR)
- Proficiency with MS Office Suite (Word, Excel, and Outlook)
- Bilingual English/Spanish language skills
