Job Summary: To coordinate the patient centered medical home, collaborating with clinicians and multidisciplinary team members by providing managerial, training and clinical support in the day to day operations of nursing within the clinic. The Nurse Coordinator will assure patient access and be a role model to ensure that quality care is provided. Must foster a culture of patient and family centered care, service, quality, and safety excellence while improving patient and employee experience and community
Knowledge, skill and ability:
- Current LPN or RN licensed in Colorado
- Intermediate level knowledge of Microsoft Office, Word, Excel, Outlook
- Strong oral/written communication.
- Ability to prioritize work and manage time
- Bilingual preferred
Education or Formal Training: Associate degree required or related experience equivalent to.
Blood/Fluid Exposure Risk:
Category I: Tasks routinely involve a potential for mucous membrane or skin contact exposure to blood, fluids or tissue. Use of personal protective equipment (PPE), when appropriate, is required.
