The Client Services Coordinator oversees clinical aspects of programming at group residences. Work in collaboration with the program coordinator, family members and other program staff to ensure that individuals living in the residence are empowered, lead self-directed lives, and have the opportunity to fully participate in their community.
- Maintain and update comprehensive functional assessment for residents.
- Ensure that day treatment programs follow ICF guidelines.
- Act as liaison for parents and community programs such as job sites, day treatment, and schools.
- Create annual social assessment.
- Act as an advocate for client.
- Document in a timely fashion all meetings concerning client.
- Coordinate clinical support staff: psychologist, speech therapist, nutritionist, etc.
- Provide relevant training for clients.
- Act as a role model for other residence staff and clients.
- Adhere to the philosophy of Toomey Residential and practice behaviors consistent with the National Alliance for Direct support Professionals (NADSP) Core Competencies, which include: Putting Individuals First; Building and Maintaining Positive Relationships; Demonstrating Professionalism; Supporting Good Health; Safety; Supporting the individual to live in the home of their choice, and being active and productive in their community.
- Maintain guidelines and policies as established by the funding sources and Toomey Residential and Community Services.
- Represent the agency in a positive and professional manner.
Schedule: 10 hours per week (Flexible)
Bachelor degree and two to five years of relevant experience required. Experience working with individuals with developmental or intellectual disabilities preferred. A valid NYS driver’s license and reliable transportation; registered and insured vehicle. Must be able to use personal vehicle for work purposes. NYS Central Registry Clearance, NYS Justice Center Fingerprinting and background check.
