Home Care Assistance is a top-rated provider of in-home care for seniors in the greater Albuquerque area. We employ a professional staff of 17 and a team of 200 direct care staff. We provide the most comprehensive plan for home care and are the only provider to integrate formal cognitive interventions into our Client Care plans using the Cognitive Therapeutics Method.
Job Summary
The Client Care Manager is responsible for all direct care staff assignments and communication for assigned clients. They are tasked with selecting and staffing qualified direct care staff for the client's service schedule and all follow-up. They will conduct monthly client home visitations, and direct care staff observations, update the client's plan of care as necessary, and contribute to the performance reviews of all direct care staff. Communication with clients and families is a key factor in the success of this position.
Work Schedule
Monday-Friday 8:00am-5:00pm; occasional weekends as necessary; weekday evening on-call rotation
Responsibilities and Duties

  • Supervises direct care employees by setting expectations for attendance, performance, and conduct by holding employees accountable to the company's policies and procedures.
  • Conducts monthly client home visits as required.
  • Creates monthly client schedules, manages changes to client schedules due to client requests, directs care staff illness, vacation, or leaves of absence, and provides alternate coverage to ensure the client's plan of care is followed and client services are not interrupted.
  • Provides thorough, investigations on all escalated client complaints and incident reports.
  • Maintains confidentiality at all times due to access to sensitive information.
  • Works independently and efficiently with the ability to apply priority management skills to complete daily tasks
  • Participation in on-call rotation

Qualifications and Skills
  • High school diploma or GED required. A degree held in a field related to Aging Life Care™ / care management is preferred
  • 2+ years of experience managing 20 or more field employees
  • 4+ years of experience managing customer expectations /relationships
  • Experience in the geriatric care industry is required
  • Strong computer & phone app skills; ability to learn quickly in a fast-paced environment
  • Professional communication skills both written and verbal; ability to effectively communicate with a variety of people at all levels of professionalism
  • Valid driver's license; ability to frequently travel within the service area
  • Bi-linguals encouraged to apply
