Position Summary:
The Hospice Chaplain is responsible for the delivery of pastoral care to patients, their loved ones, and, as appropriate, staff.
She/he will have responsibility to ensure that religious and spiritual needs of patients and their loved ones are met in areas
to which she/he is assigned.


Qualifications:
1. Must have endorsement of good standing from a personal religious leader.
2. Licensed driver with automobile insured in accordance with state/Agency requirements. Good driving record and
reliable transportation.
3. Current CPR certification.


ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY
1. Participate in delivering and arranging for appropriate pastoral care to patients/residents, their loved ones and staff
when necessary.
2. Actively participate in the delivery of pastoral care.
3. Respond to requests for pastoral care in a timely manner.
     a. Complete initial spiritual assessment within 5 days of admit.
     b. Attend admissions to services when requested.
4. Participate in the offering of worship opportunities when needed.
5. Support an interdisciplinary approach to pastoral care.
     a. Participate in team meetings.
     b. Participate in IDG at least every 15 days.
          i. This may include providing a short spiritual thought or message
          ii. Promote an interdisciplinary approach to pastoral care among all chaplains.
6. Serve on committees as appointed.
7. Participate in maintenance of proper administrative procedures for the department in accordance with institutional
policy. Follow Agency policies and procedures.
8. Keep records of pastoral activity to communicate with other members of the Agency and report to Social Services
Director.
     a. Follow agency policy for timeliness of charting. Regular notes to be turned in within 72 hours or 3 days of the
visit performed.
9. Participate in the department’s plan for continuous quality improvement.
10. Participate in staff meetings.
11. Participate in regular professional development activities and supervision.
12. Ability to work with diverse population and individuals of various different faiths.
13. Must be able to work with individuals facing end of life.
14. Must be able to work effectively in stressful situations while maintaining a calm demeanor.
15. Must be able to work independently and be flexible when faced with multiple priorities.
16. Exhibit strong interpersonal skills and be friendly, compassionate, and approachable.


TYPICAL PHYSICAL DEMANDS
1. Some direct contact with patients with communicable diseases using accepted universal precautions.
2. Substantial time spent with staff, patients and their loved ones requiring a high degree of emotional energy and mental
alertness.
3. Must have mobility to access patient’s place of residence.
4. Ability to use standard office equipment and software proficiently.


TYPICAL WORKING CONDITIONS
Majority of work is performed in providing patient care services in the patient’s home or place of residence. Exposure to
communicable diseases is common.


PERFORMANCE REQUIREMENTS
Knowledge of organizational policies, regulations and procedures to administer patient care. Knowledge of common safety
hazards and precautions to establish a safe work environment. Skilled in applying and modifying the principles, methods
and techniques to provide on-going patient care. Skilled in identifying problems and recommending solutions. Skilled in
preparing and maintaining records, writing reports, and responding to correspondence. Skilled in developing and maintaining
department quality assurance. Skilled in establishing and maintaining effective working relationships with patients, medical
staff and the public. Ability to maintain quality control standards. Ability to react calmly and effectively in emergency
situations. Ability to interpret, adapt and apply guidelines and procedures. Ability to communicate clearly.
