Healthcare for the Homeless
The Health Care for the Homeless (HCH) Program of Manchester at Catholic Medical Center, is made possible through generous support from the Manchester Health Department and Catholic Medical Center. All persons who are experiencing homelessness, or who are at risk of being homeless, are welcomed. No one is turned away due to an inability to pay.
DESCRIPTION
The Outreach/Street Medicine Team Certified Recovery Support Worker/Community Health Worker (CRSW/CHW) is a member of the HCH integrated street outreach team. Committed to delivering low-threshold, high-quality Recovery Support and other supportive services in various non-traditional settings, the CRSW/CHW will provide both acute and continuous care to individuals experiencing street homelessness. Utilizing all HCH resources and in collaboration with community partners, the CRSW/CHW will also connect patients to more traditional HCH clinic services when appropriate and facilitate patient access to other social service supports. Provides peer support to patients, assisting them with access to services and achievement of goals toward sustained recovery.
PRINCIPAL DUTIES AND RESPONSIBILITES:
Under the general guidance of the HCH Practice Manager and the direct supervision of the Behavioral Health/Outreach Coordinator, in accordance with the Federal HRSA/BPHC Program expectations, HCH program scope of practice and within established practice policies and procedures, the incumbent performs the following essential functions:
1. In conjunction with the Behavioral Health/Outreach Coordinator and as member of the HCH integrated street outreach team, participates in multidisciplinary outreach and case-finding, responding to referrals from many sources, to meet and engage with persons who are homeless and in need of HCH care, with a focus on patients who have experienced, or are active in their substance use disorders
2. Provides street based outreach, navigation, and Recovery Support to individuals experiencing homelessness within the Manchester Service Area
3. Provides Substance Use Disorder (SUD) screening and Recovery Support interventions to patients experiencing homelessness, including those with co-occurring mental health disorders, under the supervision of the Behavioral Health/Outreach Coordinator
4. Provides support and assistance to clients, in the field and at HCH clinic sites, with accessing community resources, including helping with public transportation and escorting patients to appointment, as needed
5. Links clients to appropriate emergency, transitional, and permanent/permanent supportive housing programs
6. Advocates and mediates on behalf of individuals to help them obtain and keep health care benefits, entitlements, etc.
7. Provides linkage support, coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such physical health care, money management, grooming and hygiene, meal preparation, public transportation and other independent living skills
8. Monitors patient status and response to ongoing recovery-related HCH care, community referrals, and case management
9. Collaborates with local hospital and emergency room staff, as needed, to address any recovery-related inpatient, discharge planning, and community placement needs of patients experiencing homelessness
10. Assists patients to maintain linkages with care providers and partner agencies, promoting adherence to plans of care for achievement of recovery goals
11. Supports patients in their efforts to sustain recovery, by assisting them with adherence to harm reduction and relapse prevention strategies, mental health care regimens, and medication therapies
12. Assists patients to access to SUD and MH care and treatment programs
13. In conjunction with Behavioral Health/Outreach Coordinator and as member of the HCH team, participates in planning strategies to facilitate communication, referrals, and transitions of care, in partnership with patients experiencing homelessness
14. Utilizes evidence-based practices in service delivery such as motivational interviewing, harm reduction, supported employment and education, intensive case management, housing first, and critical time intervention
15. Builds collaborative partnerships with existing and new community resources and providers for linkage and support purposes, including other community outreach teams that serve people experiencing homelessness
16. Completes all electronic medical record documentation and maintains all paperwork and other required documentation as required for program compliance and accountability in accordance with HCH standards and funding source requirements
17. Respects the dignity and confidentiality of patients in all verbal and written communication, and displays understanding of and adherence to HIPAA and 42 CFR Part 2 confidentiality laws
18. Participates in staff meetings, Quality Improvement (QI) discussions, strategic planning sessions, and program evaluations to assist with HCH program development
19. Serves as a resource to the HCH team members during integrated case management meetings
20. Reads current research, attends classes, workshops or seminars and participates in other professional development activities in order to maintain and enhance personal and professional skills
21. Provides training and consultation to the HCH team and collaborating agencies on Recovery Support and SUD issues and their intersection with homelessness
22. Maintains awareness and cultivates knowledge of resources and initiatives within the community in response to the SDOH needs of patients experiencing homelessness
23. Maintains professional ethics and boundaries; adheres to the philosophy of HCH program; complies with federal grant policies and HCH program expectations; and abides by hospital policies and procedures, to include laws governing patient confidentiality
WORK SCHEDULE:
• Work schedule will include 8 to 12 hours per day with weekend and evening coverage based on agency needs and seasonal fluctuations.
• Occasional requirement to cover special events.
Qualifications
Education:
• High School Diploma
Certified Recovery Support Worker (CRSW) preferred
Experience:
• Demonstrated at least Two (2) years of sustained recovery
• Experience and/or willingness to work with vulnerable populations
• Experience with and/or understanding of Mental Illness and Substance Use Disorder
