Certified Recovery Specialists (CRS) provide non-clinical, peer-based recovery support services to CleanSlate patients. The primary function of the CRS is to establish a relationship and build rapport with patients in order to walk alongside them in their recovery journey and toward the life they desire. CRS's draw from their own lived experience of a substance use disorder and recovery to uniquely connect with patients and provide emotional, informational and instrumental support. CRS's help individuals gain access to available resources in the community that can aid in their recovery by bridging gaps between patient needs and community resources. CRS's in turn spend a good deal of their time in the community with patients, providing support and companionship to patients recovering in the context of community and connecting to relevant resources. CRS's also conduct assertive outreach to engage new individuals into care. CRS's work collaboratively with the CleanSlate treatment team and community partners to ensure cohesive patient care.
- Collaborates with patients, physicians, office staff, ancillary departments within the company, and community agencies to maintain accurate and timely data exchanges between all entities involved in the patients' care.
- Provide peer support services to identified patients; making referrals to appropriate community resources to assist clients in dealing with the barriers they face, related to addiction; such as counseling, financial planning, housing issues, legal assistance, transportation, etc.Additionally, there will be a warm hand-off to Care Coordinators located within the CSC office as appropriate.
- Meets with and assesses each new patient for needs and barriers; obtains appropriate releases with each referral that is completed;maintains compliance with all levels and forms of patient confidentiality.
- Coordinates patient appointments, referrals, and transportation needs for treatment within the community, including but not limited to: facilitating placement for needed alternative or higher level of care as agreed upon with the patient and providers.
- Maintains effective, pro-active, and superior follow-up communication with patients, providers, office staff, and ancillary departments within CSC and community.
- Enters all required data on CSC patients into the EMR system in a timely manner and support Manager in compilation of monthly report as needed.
- Attends regular meetings or conference calls, including but not limited to involving Interdisciplinary Teams and collaborating community agencies. Provide crisis intervention, as necessary.
- Maintain individual schedule of assigned patients
- Schedule may include evenings/weekend coverage and is expected that the CRS will provide on-call coverage for a week at a time on a regular schedule.
