POSITION SUMMARY: A CNA is responsible for assisting participants in the program for all inclusive care for the elderly (PACE) with activities of daily living (ADLs), such as showering, eating, dressing, etc. They also assist in the delivery of instrumental activities of daily living (IADLs) and restorative care, such as accompaniment to clinic appointments, physical therapy exercises and quality of life activities. CNAs are instrumental in the fulfilment of our mission to keep participants living as independently as possible for as long as possible and function as an important part of the interdisciplinary team (IDT).

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
- Assists participants with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed
- Creates and participates in group(s) and one-on-one activities in memory impaired unit
- Monitors and assists participants with meals at feeding table
- Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas
- Assists the drivers with the loading and unloading of participants from vehicles
- Demonstrates dependability through consistent compliance with scheduled work hours
- Participates in participants care planning through cooperation with interdisciplinary teams
- May assist other departments as a CNA as approved by the Day Center Supervisor
- Assist with Meal Reimbursement sheets as needed
- If Wellness: provides wellness programming as directed by the rehabilitation department upon discharge from skilled therapy
- If Wellness: Instructs and supervises participants performing their exercises. Acts as a support to participants
- Documents services provided to participants consistently, accurately and in a timely manner via Electronic Health Record (EHR). Follows procedure for flow sheet completion
- Participates in participants care planning as requested
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices universal precautions
- Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.
- Participates in Quality Assurance and program improvement initiatives as assigned

QUALIFICATIONS AND REQUIREMENTS:
- High School Diploma or equivalent. Must be licensed as a Certified Nursing Assistant (CNA) in the state of Colorado
- Minimum of one (1) year of experience working with a frail, elderly or disabled population is required.
- Current First Aid/BLS Certification is required
- Must have excellent written and verbal communication skills and demonstrate the ability to interact clearly and effectively with both internal and external customers
- Must be efficient with computer applications such as email, navigating the internet, using electronic timekeeping, and Electronic Health Records
- Ability to work independently with minimum supervision
- Knowledge of how to use and maintain medical equipment (adaptive, hoyer lift, etc.) is strongly preferred

Physical Requirements While performing the duties of this job, the employee is frequently required to traverse across a variety of terrains inside an office environment, outdoors and in participants homes (if applicable). This role will also be required to operate computer equipment, tools or other objects needed to perform the essential duties of the role. This role requires the ability to communicate effectively through written and verbal forms. This role often is required to position oneself in a kneeling, bending, or crouching position as well as ascending and/or balancing on a ladder. The employee may be required to endure extremes in temperature and may work in hazardous environments where personal protective equipment is required. Requires the ability to move boxes, files, supplies/equipment up to 50 pounds using appropriate body mechanics or push/pull 150 pounds using proper equipment and/or body mechanics.
Environmental Conditions Working conditions may be noisy with fluctuating indoor temperatures. Must have the ability to work under moderate pressure to meet scheduled appointments or deadlines while dealing with frail, disabled and/or confused participants/clients. Subject to individuals who may have the potential for physical or verbal aggression. May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, communicable diseases and other conditions common in a clinic environment. Subject to unpleasant odors.
