QUALIFICATIONS:
- Mustbe at least 18 years of age
- Membershipon the Nurse Aide Registry, as monitored by the PA Department of Health.Certification must be active and in good standing
- Oneyear of experience in Long Term Care preferred but not required
RESPONSIBILITIES INCLUDE:
- Assistsin activities of Daily Living needs of the resident - i.e., bathing, dressing,hair care, mouth care, shaving, toileting, nail care. Provides incontinence careand changes bed linens
- Assistswith daily and ongoing activity programs for all residents
- Lifts,turns, and transfers residents, as scheduled or as needed. Properly usesmechanical lifts and require 2 staff for all transfers
- Observesand reports any change in resident's condition to LPN or RN using Stop andWatch in EMR. Completes and documents all areas of care in the EMR system.Weights/reweighs, BM records, restraint records (when applicable) input andoutput and meal percentages, ADLs, mood/behavior, toileting
- Providescare in a manner that maintains resident's dignity and state of well-being
