Responsibilities and Duties

• Work with a registered nurse and other clinicians to provide care based on an individualized plan of care.
• Record your patient’s health status and report it to the nurse.
• Provide companionship and personalized care while encouraging independence and self-care.
• Assist patients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
• Provide mobility and transfer assistance, which may include using mechanical lift devices.
• Prepare meals and snacks. May include grocery shopping.
• Perform light housekeeping, including vacuuming, laundry, and linen changes.
• Assist with incontinence care.
• Assist with transportation needs.
• Staffing opportunities available, facility based.

Required Qualifications and Skills

• Current and valid Certified Nursing Assistant (CNA) or Home Health Aide certification
• CPR certification
• Ability to lift at least 40 pounds
• Availability to work on weekends, evenings, and holidays
• Reliable transportation
• Valid driver’s license and insurance
• Must be available weekend.
• Must be able to pass a background check.
• Must have a clean driving record.
• Must have a TB Clearance
• Must have first aid /CPR
• Prior Home Health experience preferred, will consider facility based experience

Salary and Benefits

• $18.50-24.00 per hour based on certification and experience
