AHC-American Health Communities is a network of 29 skilled nursing and rehab facilities. We treat patients recovering from illness, injury or surgery and provide daily care for long-term residents. ESSENTIAL JOB DUTIES:

• To perform this job, an individual must accomplish each essential function satisfactorily, with or without a reasonable accommodation.
• Trains under the supervision of professional staff on how to provide personal, hygienic care for patients, such as bathing, hair, mouth and skin care
• Trains on how to check vital signs, i.e. temperature, pulse and respiration.
• Trains on how to perform uncomplicated treatments as assigned and instructed by the professional and as allowed per state regulations
• Trains on how to provide light housekeeping and/or homemaker services. Services may include light housekeeping, linen changes, laundry, meal preparation and companionship
• Trains on how to assist with physical activity as assigned, such as transferring, ambulating, simple exercises and encourages independence
• Trains on how to remind the patient to take their medications as prescribed by the physician
• Trains on how to assist the patient and/or family in providing a clean, safe environment
• Trains on how to prepare simple meals and assists patient with eating when needed
• Trains on how to establish a relationship with the patient and family that promotes trust and provides emotional support
• Trains on how to record observations of treatment and care given and reports any significant change in the patient's condition or problems to the supervisor
• Trains on how to submit all documentation in a timely manner according to agency policy
• Receives instruction regarding mandatory attendance for twelve hours of in-service per agency policy and procedure
• Reviews the company's policy and procedures
• Trains on how to ensure protection of confidential information that is stored, handled or maintained by following Company Policies and Procedures related to confidentiality and/or HIPPA regulations
• Other duties as assigned

REQUIRED QUALIFICATIONS:

• Education:
• High school diploma or GED required
