JOB DESCRIPTION:

• Hours: Monday-Thursday from 3p-9p
• Home Care Setting
• Assess patient’s needs and provide individualized care.
• Assist with necessary activities of daily living.
• Must be able to use electronic documentation and understand how to submit documentation electronically.

REQUIREMENTS:

• At least one year of home care experience
• BLS Certification
• Cleared background check.
• Active driver’s license
