Essential Job Functions
- Teaches and assists clients in completing tasks and activities.
- Implements program goals for each individual.
- Plans, organizes, and implements community outings and related daily activities.
- Participates in annual program plan reviews for assigned individuals in the program.
- Plans and implements activities according to each individuals identified strengths, needs, and desires as identified on his/her program plan.
- Completes all necessary documentation on a consistent basis.
- Responsible for the daily care, supervision, safety, and training of assigned individuals.
- Participates in on-going in-service training and participates in at least 24-hour in-service training annually including, but not limited to First Aid Training, personal emergency interventions, Fire Safety, and other safety techniques.
- Responsible for proper implementation of de-escalation techniques and authorized restraint techniques in accordance with the emergency restraint policy.
- Respects each individual’s limitations and assists each individual to make choices.
- Promotes good social interactions with each individual that is served.
- Provides each person with as much assistance as necessary to complete tasks.
- Communicates with the Director, Supervisors, Coordinators, and Program Specialists/QIDPs about concerns arising in the care and training of assigned individuals.
- Takes an active interest in implementing, promoting, and protecting the civil rights of each individual.
- Maintains confidentiality relative to the individual’s records, training progress, and tracking documentation according to HIPAA regulations.
- Promotes a safe and clean working environment.
- Responsible for maintaining a positive work atmosphere by behaving in a professional manner and promoting camaraderie with co-workers, supervisors, and others.
- Shares responsibilities in monitoring breaks, lunches, and van duty.
- Responsible for consistent punctuality and attendance.
- Follows all work rules and policies for the Developmental Programs Division.
Other Job Functions
- Assists nursing with medication administration and documenting on the MAR or the eMar for individuals at the Day Program.
- Assists nursing with tube feedings, bolus feedings, and tube flushes.
- Assists nursing with unhooking and hooking feeding tubes before and after individual’s toileting.
- Assists nursing with monthly MAR’s, pulling old documentation and filing updated information.
- Assists nursing with medication expiration checks and compiling updated lists for pharmacy.
- Assists nursing with maintaining supply management, both supplemental and nursing supply orders.
- Assists nursing with problem solving issues as they arise.
- Performs other duties as assigned by Administration and the Day Program management team.
Knowledge/Skills/Abilities

- High School diploma required.
- Certified Nursing Assistant (CNA) is required.
- Minimum of one (1) year of experience in the healthcare field is required.
- Ability to perform a one person pivot as well as assisting individuals to bear weight and ambulate with or without assistance is required.
- Can demonstrate the ability to perform a two-person lift and transfer of individuals to and from their wheelchairs.
- Ability to read and write competently in English.
- Ability to carry out instructions furnished in written, oral, or diagram form.
Certificates and Licenses:
- Valid (state of residence) Driver's License and driving approval from the Archdiocese of Philadelphia is required.
- Ability to successfully pass and maintain acceptable background checks – FBI, PA Child Abuse, and PA State Police Criminal History.
Supervisory Responsibilities:
This position does not have any direct supervisory responsibilities.
Work Environment:
- While performing the duties of this job, the employee will be required to work in a variety of settings and environments and has an occasional exposure risk to adverse weather conditions.
- The noise level in the work environment is usually light to moderate and the office is subject to frequent interruptions throughout the day.
Physical Demands:
- The employee must regularly lift and /or move up to 10 pounds, frequently lift and/or move up to 50 pounds and occasionally lift and/or move more than 100 pounds with assistance.
- Specific vision abilities required by this job include Close vision, Distance vision, Peripheral vision, Depth perception and Ability to adjust focus.
- While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; reach with hands and arms and talk or hear. The employee is frequently required to walk and sit.
- The employee is occasionally required to climb or balance; stoop, kneel, crouch, or crawl and taste or smell.
