Traditional Home Health Care is hiring compassionate, dedicated, Certified Nurse Assistant (CNA)/Home Health Aide, HHA to provide one-on-one care to our patient located in Monroe County.

Job responsibilities include, but not limited to the following:
- Performs personal care/ household duties (e.g. dressing, toileting, feeding, light housekeeping) in home/school setting
- Maintain a clean and safe environment
- Provide emotional/behavioral support
- Follows plan of care
- Ability to interact as a member of a multidisciplinary team
- Reports any changes in the patient condition to agency/nurse case manager
