Under the direction of the collaborative clinical team lead, performs clinical services relating to the review, evaluation and management of drug therapy to a patient, being treated by a physician for a specific disease or associated disease states, in accordance with a written agreement or Protocol with a voluntarily participating physician.

1. Operates under defined protocols based upon agreement(s) with Collaborating Provider(s) and in accordance with the policies, procedures and protocols of NYC Health + Hospitals (H + H) and New York State (NYS) guidelines.

2. Implements and modifies drug therapy of individual patients or groups of patients (patients with diabetes, asthma, hypertension, etc.).

3. Adjusts the management of drug strength, frequency or route of administration and/or substitutes authorized drugs in accordance with the Drug Therapy Protocol with the Collaborating Provider.

4. Evaluates clinical laboratory tests related to the drug therapy management for the specific disease or disease state specified within the Protocol and orders such clinical laboratory tests, only if specifically authorized by the protocol and to the extent necessary to discharge their responsibilities under the law.

5. Orders and/or performs routine patient monitoring functions, as necessary, in drug therapy management including collecting and reviewing patient histories, conducting relevant physical assessments and ordering or checking of patient vital signs, including pulse, temperature, blood pressure and respiration, to the extent necessary to discharge responsibilities set forth in the Protocol. 6. 2. Must be certified to practice Collaborative Drug Therapy Management (CDTM) by the New York State
Education Department (NYSED).
