Job description

A1 Service Coordination is a Conflict Free Service Coordination agency committed to providing the highest standard of Service Coordination to individuals and families of the Nursing Home Transition and Diversion (NHTD) and Traumatic Brain Injury (TBI) Waiver Programs. Our Service Coordinators collaborate closely with each participant to create a personalized plan to help them overcome their challenges and achieve their goals. A1 Service Coordination is currently providing support to the Traumatic Brain Injury, disabled, and elderly community members and their loved ones in the New York City and Lower Hudson Valley Regions. We are currently looking for professional, dedicated, and motivated members to join our team!

Positions Available:

1. Full-Time On-Going Service Coordinator located in NYC Region:Must be willing to travel to and from participants located in Manhattan, Bronx, Queens, Brooklyn, & Staten Island. Field Work with in-person home and/or community visits required for this position. Bilingual Speakers are preferred but not required (Language preferences Russian, Spanish, Cantonese, Mandarin, & other).

2. Full-Time On-Going Service Coordinator located in Lower Hudson Valley Region:Must be willing to travel to and from participants located in Westchester & Putnam County. Field Work with in-person home and/or community visits required for this position. Bilingual Speakers are preferred but not required (Language preferences Russian, Spanish, Cantonese, Mandarin, & other).

On-Going Service Coordinator Job Summary: An On-Going Service Coordinator aids individuals enrolled onto the NHTD/TBI waiver program. An On-Going Service Coordinator is responsible for completion of all tasks related to the development and submission of an On-Going Service Plan. An On-Going Service Coordinator can have a maximum active caseload of twenty-five (25) participants.

3. Full-Time Initial Service Coordinator: Must be willing to provide support to applicants in the New York City and Lower Hudson Valley Region. Field Work is not required for this for this position. Bilingual Speakers are preferred but not required (Language preferences Russian, Spanish, Cantonese, Mandarin, & other).

4. Per-Diem Initial Service Coordinator: Must be willing to provide support to applicants in the New York City and Lower Hudson Valley Region. Field Work is not required for this for this position. Bilingual Speakers are preferred but not required (Language preferences Russian, Spanish, Cantonese, Mandarin, & other).

Initial Service Coordinator Job Summary: An Initial Service Coordinator helps individuals seeking enrollment onto the NHTD/TBI waiver program.
