What is this job?

Position Summary:

The Case Work Specialist acts in a dual role, supporting both the case management and therapeutic services for all youth. The Case Work Specialist will be the primary point of contact for family members and will deliver clinical intervention, including but not limited to individual, group and family therapies. All of BCC’s Case Work Specialist are credentialed and certified trauma professionals within their first year at BCC. The Case Work Specialist works closely with all members of the care team to design and implement guidance and treatment plans for the youth in order to develop, support and encourage safe behaviors, self-regulation and healthy relationship.

What does this job do specifically?

Essential Functions:

  • Model and act in accordance with BCC’s core values.

  • Provide individual, group, and family therapy, within the scope of professional license, to children, adolescents, and their families.
  • Take the lead in being the primary contact for youth’s identified supports.
  • Learn and implement evidence-based practices selected by the organization, which may include attending on- or off-site training, participating in ongoing supervision, consultation, and/or continuing education, committing to delivery of the practice as expected by BCC, following practice guidelines with fidelity, and engaging in fidelity monitoring.
  • Provide effective management of assigned cases which includes but is not limited to: developing, monitoring, and reviewing Individual Service Plans, Safety Plans, and Transition/Discharge Plans, and collaborating in transition/discharge planning.
  • Serve on the Interdisciplinary Care Team for each youth served, to include consultation with the providing psychiatrist regarding client needs and interventions, and working collaboratively with Treatment Team Manager, Assistant Program Director, Unit Supervisor, Campus Supervisors, Youth Care Professionals, Lead Staff, and Medical Assistant.
  • Evaluate the child’s/youth’s/family’s progress towards goal attainment on a designated basis in line with individual treatment and transition/discharge plan goals that are individualized, have strengths and service needs clearly identified, and are developed in behaviorally measurable terms.
  • Complete psychosocial intake assessments and ongoing clinical assessments that include participation by child/youth and parents/guardians.
  • Provide clinical services that are trauma-informed and aligned with BCC’s purpose and values.
  • Coordinate with the Board of Child Care staff members and community resources to address clients’ service needs.
  • Maintain effective working relationships with families, children/youth, colleagues, consultants and community agencies to include scheduling home visits, notifications of serious incidents, follow-up on all ongoing documentation needs.
  • Provide effective case management to assigned cases which includes but is not limited to liaison with internal and external professionals such as health professionals, schools, placing agencies, lawyers, community partners, and other human service agencies.
  • Ensure assigned case records meet documentation standards including monthly chart audits. Submit by the 15th of each month, to the Admin Asst. documentation of the chart audit.
  • Complete required documentation and reports within the defined time lines and/or as requested and ensure that case records meet documentation standards. This includes, but is not limited to: completion of comprehensive Individual Service Plan within 30 days of admission, completion of progress reports on assigned case load on a monthly basis, updating the Individual Service Plans every six months or as needed, documentation of all contacts made regarding caseload, completion of the Initial Assessment form within five days of admission, HCSIS reports, etc.
  • Create Guidance Plans for the living environment with input from the care team and act as a consult to the care team, family, and school regarding clinical and behavioral presentations and interventions.
  • Complete required documentation and reports within the defined timelines.
  • Ensure assigned case records meet documentation standards.
  • Engage in the peer review process.
  • Record and submit billing data within the defined timelines.
  • Promote child/youth/family understanding of available Board of Child Care and community resources and facilitate referrals as appropriate.
  • Actively participate in clinical group supervision on a regular basis, ensuring there is adequate case consultation.
  • Participate in bi-weekly clinical team meetings, weekly care team meetings, departmental and program-specific interdisciplinary team meetings, as well as ITP and IEP meetings as appropriate.
  • Act as Case Work Specialist on Call on a rotating basis or maintain a schedule that includes a weekend shift to provide family engagement activities.
  • Obtain and maintain certification as a “user” in BCC’s electronic health record software system, and FamCare
  • Works with the Program Coordinator to obtain information on incoming resident’s educational needs and participates in BID meetings when appropriate and ensures adherence to the ESSA law.
  • Meet regularly with school personnel to address ongoing issues related to school performance and behavior.
  • Provide support complicated IEPs and ISPs and other educational needs of the youth.

What shifts would I work? Are full time positions available?

Position Type/Expected Hours of Work:

This is a full-time position. Standard days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m., and must include two evening shifts (e.g. 11:30 a.m. – 8 p.m.). Must rotate through as Case Work Specialist on Call. Attendance at occasional weekend or evening family engagement events may be expected. This position is eligible for flex schedule per BCC policy and procedure. This position is essential during inclement weather.

What experience and education do I need?

Required Education and Experience

  • Master’s Degree in Social Work, Counseling, or a related field from an accredited college or University
  • Current LGSW, LGPC, LCSW-C, LCPC, LGMFT, LCMFT, LGPAT, or LCPAT Pennsylvania State License
