Position Summary

Responsible for assessing, planning, coordinating and evaluating the care of patients.

Job Description

Primary Duties & Responsibilities

  • Communicate with patients daily about the service needs, service guidelines and eligibility.
  • Communicate with medical and supportive service providers/staff daily regarding scheduling, client/patient barriers, best practices, etc.
  • Monitor inventory, determine needs, and place orders for client service supplies (i.e., food pantry, toiletries, bus tickets, etc.).
  • Track timeliness of services, services provided, report and address issues with services, trouble-shooting and resolve issues.
  • Complete service documentation in EPIC and statewide case management database.
  • Monitor and request additional supplies from vendors and ensure timely receipt and distribution.
  • Other duties as assigned.

Preferred Qualifications

  • Provision of case management and/or prevention education services.
  • Knowledge of community resources.
  • Computer skills.

Required Qualifications

Master’s degree in social work, counseling, psychology, or a related field. An equivalent combination of experience and education may substitute for a master’s degree.

Salary Range

$51,600.00 - $77,400.00 / Annually

The salary range reflects base salaries paid for positions in a given job grade across the University. Individual rates within the range will be determined by factors including one's qualifications and performance, equity with others in the department, market rates for positions within the same grade and department budget.
