Pay Rate: $27.47 an hour.


Basic Function/Overall Responsibilities: The Case Planner/Case Aide Supervisor will assist with training and supporting Case Planners and Case Aides with their job duties. Maintain a caseload of high needs clients, to include assisting with implementation of family assessment service plans and role modeling/facilitating productive working relationships between families, agency personnel, and other involved external stakeholders. The Case Planner/Case Aide Supervisor is responsible for oversight in the development of the visitation schedule completed by the Foster Care Program Assistant. They will also be responsible for finding visitation coverage in case of staff absence or emergency. They are to ensure the proper and timely documentation of all visitations. The Case Planner/Case Aide Supervisor will implement the Sanctuary Model of care into visitation while providing guidance to the families in their areas of needs to meet their permanency planning goal. Flexible work schedule: non-traditional working hours required and will be included in the secondary on-call rotation.

Principal Assigned Responsibilities:

  1. Implementation of family assessment service plans; provide in-home services including monitoring of provisions and safety, behavior management training for families and supervise visits between children and families when necessary.
  2. Availability to provide periodic on call crisis intervention to families per a shared on-call rotation, as needed.
  3. Assist families in identifying and obtaining appropriate clinical, recreational, financial, medical or other support services designed to fully integrate the family with community-based supports in accordance with treatment plan; make referrals as necessary.
  4. Monitor school attendance and performance for each child on caseload in accordance with treatment team recommendations.
  5. Act as a liaison between parents/families, Family Court, Probation, school officials, agency personnel or other service providers as necessary/appropriate.
  6. Actively participate in treatment team meetings and planning conferences.
  7. Complete documentation per agency policy and procedure (e.g. log entries, mileage, medical, incident reports, intake, Family Assessment Service Plans, Plan Amendments, affidavits, utilization reviews, service plans, etc.)
  8. Actively participate in Serviced Plan Review meetings and Case Conferences to which they are invited, to provide additional support.
  9. Attend agency in-service trainings, workshops, and seminars as appropriate or required by supervisor.
  10. Provide weekly supervision to the Case Aides and lead weekly team meetings with team members when program manager is out.
  11. Ensure that all visitation requirements can be met through individual weekly schedules of the weekly Case Aides and respond to emerging situations as they arise.
  12. Manage the referral process for cases that need Case Aide’s services.
  13. Ensure Coaching Family Visits (or other model utilized) training is occurring to fidelity standards and provide direct oversight and supervision to staff engaged in such.
  14. Ensure that all Case Aides follow appropriate and timely documentation standards for visitation.
  15. Operate as the point of contact for your assigned staff and the cases and stakeholders involved with such, to include but not limited to, birth parents, foster parents, community partners and County DSS partners.
  16. Ensure that all visitation rooms have the necessary supplies and furnishings as well as ensuring compliance with infection control policies and protocols.
  17. Other associated tasks deemed appropriate by supervisor.

Education and Experience Requirements:

1. Bachelor’s degree, preferably in human services or a related field required.

2. Minimum of three-years successful human services experience working as a case planner, case worker, care coordinator or similar care management experience required.
