We assist families caring for children that are medically fragile, chronically ill or living with a life threatening illness. Our Nonprofit has been around for 31 years and was founding by a parent to help families.

Responsibilities:
- Conduct comprehensive assessments of patients' needs and develop individualized care plans
- Coordinate and facilitate communication between patients, families, and healthcare providers
- Monitor and evaluate patients' progress towards achieving their goals
- Collaborate with interdisciplinary teams to ensure continuity of care
- Advocate for patients' rights and access to appropriate resources
- Provide crisis intervention and support to patients in critical care, ICU, ER, and behavioral health settings
- Document all interactions and interventions in a timely and accurate manner
Requirements:
- Bachelor's degree in related field
- Experience working in health settings
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Monthly Home visit in Nassau or Suffolk County NY
- Excellent problem-solving and decision-making abilities

Pay: $24.00 - $27.00 per hour
Expected hours: 40 per week
