Under the general direction of a Case Management Supervisor, responsible for the full range of case management duties including cases requiring intensive case management and high levels of expertise. Implements the clients' service plans. If assigned to support Young Children (ages 3-6), will be responsible for assisting Intake with the process of determining Lanterman eligibility for children under “Provisional Eligibility” criteria prior to turning 5 years old.

MINIMUM EDUCATION AND EXPERIENCE REQUIREMENTS

A Master's degree in a field related to developmental disabilities.

OR

Bachelor's degree in a field which offers content related to developmental disabilities, such as social work, psychology, counseling, a special education or nursing and three years employment in a field related to developmental disabilities.

Minimum qualifications may be waived depending on the agency’s need for specialized skills (such as language abilities).

• For those speaking a language other than English - $175 per month

Pay: $59,244.00 - $79,428.00 per year
