Summary:
This position will provide case management services to program participants. The Case Manager provides direct services to particpants including the completion of needs assessments, care plans, periodic reassessments and overall comprehensive service coordination. This position reports to the Program Director.
Essential Duties and Responsibilitites:
Core duties and responsibilities include the following. Other duties may be assigned.
· Provide case management and on-site crisis intervention.
· Faciliate the intake and assessment process and develop care plan.
· Educates participants with respect to good housekeeping and home-safety practices, budgeting and landlord/tenant relations, as necessary.
· Maintains appropriate documentation of all tenant contacts.
· Documents monthly progress notes in the participants’ chart.
· Facilitate on-site support groups and recreational activities.
· Conducts home visits to help evaluate participants’ compliance with the rules and regulations of the program and to assess the need for referral to supportive services.
· Acts as liaison between participants and building managers to ensure all repairs are completed to either the participant’s apartment unit or the building.
· Provide holistic and comprehensive case management services.
· Maintain knowledge of community resources and counseling/social work practices with high risk populations.
· Connect participants with needed and available community resources.
Qualifications:
· Master’s degree in social services or related clinical field and 2 years’ experience working with applicable populations; or
· BS/BA Degree in a human services related discipline and 5 years’ experience working with applicable populations.
· Experience working with homeless populations and those with SMI and/or SUD
· Strong interpersonal skills, including focusingon solving conflict, not blaming, maintaining confidentiality, listening to others without interrupting, keeping emotions under control, and reemaining open to others’ ideas.
