Case Manager

```Overview``` We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing and implementing care plans, and connecting clients with appropriate resources and services. If you have a passion for helping others and possess strong interpersonal skills, we encourage you to apply. ```Duties``` - Conduct comprehensive assessments of client needs and develop individualized care plans - Provide ongoing case management services to clients, including regular check-ins and monitoring progress - Connect clients with community resources and services, such as childcare, workforce development programs, and Section 8 housing assistance - Supervise and coordinate services provided by other professionals involved in the client's care, such as therapists or counselors - Provide crisis intervention and support during times of emotional or behavioral distress - Utilize motivational interviewing techniques to encourage clients to set goals and make positive changes in their lives - Collaborate with other professionals, such as social workers or healthcare providers, to ensure holistic care for clients - Maintain accurate and up-to-date documentation of client interactions and progress ```Requirements``` - Bachelor's degree in social work or related field (Master's degree preferred) - Previous experience in case management or related field - Knowledge of social service programs and community resources - Strong communication and interpersonal skills - Ability to work effectively with individuals from diverse backgrounds - Familiarity with crisis management techniques and addiction counseling principles - Proficiency in motivational interviewing techniques - Ability to handle sensitive information with confidentiality and professionalism If you are passionate about making a difference in the lives of others and possess the necessary skills and qualifications, we invite you to apply for the position of Case Manager.

Pay: $56,119.00 - $62,750.00 per year
