ABOUT THE JOB
MU Health Care is looking for a passionate and dedicated Case Manager for Therapy Services to join our dynamic team. The ideal candidate will possess excellent communication skills, both written and verbal, demonstrating the ability to interact effectively with patients, families, and a multidisciplinary healthcare team.
As a Case Manager, you will play a crucial role in ensuring the seamless continuum of care for our patients. Your responsibilities will include conducting comprehensive social evaluations, providing counseling for illness adjustment, and offering emotional support to patients and their families. We value candidates who stay up-to-date with community resources, actively participate in developing community relationships, and coordinate transition care for patients and families. Your expertise will contribute to facilitating and coordinating patient progression through a case management approach, collaborating with healthcare providers, and organizing multidisciplinary care conferences.
The successful candidate will have a minimum of three years of clinical experience, demonstrating a deep understanding of medical terminology, patient diagnoses, treatment protocols, and disease processes. Additionally, we welcome applicants with various qualifications, such as a Master of Social Work degree, licensure as a Licensed Clinical Social Worker (LCSW), or credentials as an Occupational Therapist, Speech Language Pathologist, or Physical Therapist.

Three (3) years of clinical experience, which provides knowledge, and understanding of medical terminology, patient diagnoses, treatment protocols and disease process. When primary state of residency changes, compact state RNs must apply under new state of residency within thirty (30) days, or -Occupational Therapist (OT) licensure by the Missouri Board of Occupational Therapy, or -Speech Language Pathologist (SLP) licensure by the Missouri Commission for Speech-Language Pathologists, and Certificate of Clinical Competency (CCC) certification from American Speech-Language-Hearing Association (ASHA), or -Physical Therapist (PT) licensure by the Missouri Board of Registration for the Healing Arts or PT Compact multi-state privilege. When primary state of residency changes, compact state PTs must apply under new state of residency within thirty (30) days.
