Job Description:
- Responsible for collaborating with the members of the health care team to facilitate coordinated and cost-effective services for a defined population
- Help members achieve optimal independence and enhance quality of life
- Manage and coordinate assigned caseload to evaluate patient goals and social support systems
- Develop and monitor a plan of care to promote optimal functioning
- Advocate appropriate resources and ensure timely implementation
- Document case management activities in compliance with standards
- Analyze outcomes for program improvements
- Provide counseling and education to members, families, caregivers, and the community
- Maintain knowledge of current case management standards and Social Work ethics
- Minimum 3 years of clinical experience and 2 years in case management
- Minimum 1 year of experience with the defined population
- Masters degree in Social Work (MSW)
- Licensed Clinical Social Worker in Virginia, Maryland, and District of Columbia required
- Case Manager Certificate within 36 months of hire
- Experience with computer software programs in a Windows environment
- Knowledge of community systems and resources
- Knowledge of regulatory issues in the Mid-Atlantic area
- 16 hours per week
- Evening shift, specific days alternating every week
- Working hours: 03:00 PM - 11:30 PM
- Short Hour job schedule
- Travel required: 20% of the time
