Job Title: Case Manager - Registered Nurse (RN)
Location: Kingston, NY
Are you a compassionate and dedicated Registered Nurse (RN) seeking an opportunity to make a positive impact on the lives of individuals in need? We are currently hiring for two Full-Time positions in Kingston, NY, and surrounding areas.
Position 1: Full-Time RN Case Manager
- Schedule: 9 am to 5 pm, Monday to Friday
- Responsibilities:
- Provide comprehensive case management to individuals in need
- Administer medications in adherence to prescribed protocols
- Collaborate with healthcare professionals and support staff to ensure holistic patient care
Position 2: Full-Time Temp to Perm RN Case Manager
- Schedule: Flexible hours with a commitment to Full-Time
- Location: New Paltz and Kingston group homes
- Compensation: $45.00 per hour
Registered Nurse Responsibilities:
- Provide temporary and permanent case management services
- Collaborate with a multidisciplinary team to address the unique needs of individuals in group homes
- Ensure the delivery of high-quality healthcare services
- Administer medications as prescribed
Registered Nurse Qualifications:
- Valid RN license in the state of New York
- Proven experience in case management
- Strong organizational and communication skills
- Ability to work collaboratively in a team setting
- Compassionate and patient-focused approach
